ORDER GRANTING CHILD SUPPORT
THIS CAUSE having come on for hearing on the petition of the
DEPARTTfENT OF HEALTH AIvD REHABILITATIVE SER~'ICES for child support and the
Court having found tha[ said Petitioner is a proper party by virtue of an
assignment of rights to child support signed by the party having custody of
the dependent child(ren) and all parties having received pro er notice, it
is
ORDERED AND ADJUDGED that commencing on ~ y , 1982,
the responsible parent RUDOLPH SEARS, JR., who is the father, of th minor
child(ren), to-wit: RUDOLPH SEARS, IIIt d.o.b. 7/29/81, shall pa t t Clerk
of this Court the sum of $,~J, c~c~ per C-c~ each and every L, plus
$2.00 statutory fee. All payments shall be made in cash, money order or cashiers
ct~eck. All money orders and cashiers checks shall bear the payees name and
Social Security Number and shall be made payable to the CLERl; CF THE CIRCUIT

~ he is hereby ordered to continue to transmit support payments received from
f the Respondent to the Department of Health and Rehabilitative Services until
~ further order of this Court to the contrary or receipt of Notice to
~ Discontinue Payments from the Departr~ent of Health and Rehabilitative
~ Services, in which event the support payments shall thereafter be directed
~ and payable to the person having custody of the child(ren).
