there shall not be an excess of two weeks of child support
credit for any one visitation.
Each payment shall be made to the Clerk of this Court
and by him remitted promptly to the Wife. In addition to the
amount of child support to be paid by the Husband as more fully
set forth above, the Husband shall be solely responsible for
the payment of any Clerk's fee or statutory cost which shall be
added to each child support payment made to the Clerk. All
payments shall be made in cash, money order or cashier's check.
IT IS FURTHER ORDERED that the Husband shall promptly

I notify the Clerk of this Court of all chanqes in his mailinq

` and residence address and all chanqes in the name and address

F of his employer.
~ 4. PROPERTY SETTLEMENT AGREEMENT: The Property

s Settlement Aqreement admitted into evidence is hereby

~ incorporated herein by reference and the parties are ordered to

E comply with the provisions thereof.

; 5. RESTORATION OF WIFE'S FORMER NAME: PATRICIA LeVINESS
~ be and she is hereby authorixed to assume her former name of

k PATRICIA K. HARRINGTON and to use the same in all respects as
if said marriaqe had not intervened.
6. MUTUAL RESTRAINING ORDER: Each of the parties is
hereby enjoined and restrained from interfering with, harassinq
; or annoyinq the other and that said mutual restraining order
~ shall be enforceable by the Sheriffs of the State of Florida.
