terms as set forth herein in 7(B). Upon the expiration of the six (6)
consecutive weeks, the each parent shall resu~e his visitation schedule as
set forth hereinabove.
C. The Husband shall be responsible for the children's medical
and dental care.
D. Both the Husband and Wife shall be equally responsible for the
children's religious education, athletic develo{~ment and co1le~,e education.
E. Each Party agrees to consult the other when important
decisions must be made with regard to their areas of primary
responsibility. Each Party agrees to make no significant change in the
children's educationai ar religious ptograms without the consent of the
other Party.
F. Each Party shall notify the other promptly of the children's
serious illness or accident.
G. Each Party shall have access to the records and information
pertaining to the minot children, including but not limited to, medical,
E dental and school recQrds. If either Party obtains such records in Written
; form, that Party shall provide the other with a copy of said records within
~ fifteen (15) days.
~ H. Each Party shall be entitled to autharize emergency medical
i treatment for the minor children.

I. Each Party shall u~e all reasonable efforts to maintain free
i access and to create a feeling of affection between themselves and the
~ minor children; neither Party shall do anything to interfere with the
~ natural development of the children`s lave and affection for the other
~ Party.
~ J. The parent with whom the children actually resides shall have
; the responsibility to make daily routine decisions which are necessary in
= the natural course of caring for the children without prior consultation
; with the other parent.

~ K. The Parties agree that they will consult with one another with
~ rsgard to any claim, cause of action, award, or inheritance accruing to the
~ minor children. The Parties further agree that should there be any
recflvery or payment on account of any claim, ca~s~ of action or award,
reward, or inheritance, it shall be itnmediately placed in an
interest-bearing trust or custodial accou~t for the benefit of the minor
chi~dren to whom it inures. The money will be left in the trust fund untfl
the children reaches the age of majority. However, money nay be withdrawn
' from the account prior to the children's age majority if the Parties agree
that the children need the money for college tuition and expenses or have
another equally important need that the Parties are financially unable to
~ fulfill.
~ NEITHER PARTY shall conceal the whereabouts of the minor
children, and, further, each parent is to keep the other parent advised a*_
~ all times of the residency address and telephone numbers where the children
