I, GUY T80MAS PARTINGTON, father of the minor child, GUY
THOMAS CART, hereby relinquish all rrghts and responsibilities in
reqards ta said minor child and consent to any future adoption of
the child by an individual approved by the Court, to be his legal
child and heir at law.
It is my understanding that PAMELA MICHELE CART, in
consideration for my execution of this Waiver of Parental Rights,
waives any and all claims she may have to past, pre~ent or future
child support far said minor child. ~

; waiver, I am waiving any future rights of visitation or

~ communication with GUY THOMAS CART.
