That the above-named Defendant having been adjudicated the father of the above-named child(ren), the Department of Health and Rehabilitative Services, Bureau of Vital Statistics, Amendment Unit, shall and it is t~reby ordered to amend the abave-named child's/children's birth certificate(s) to show the above-named father's name. 6. TY~at pursuant to Section 443.051, Florida Statutes (1985) and Section 462(e) of the Title N-A of the Social Security Act that the Department of Labor and ~nployment Security shall deduct and Withhold from the Unemployment Compensation othe~ise payable to the Defendant SOX of the Ur~loyment Compensation or the amount of child support as ordered above Whichever equals the greater amount but does not exceed the court ordered support aQaunt. 7. Additionally, it is further ordered that the Respondent/Payor shall promptly notify the Clerk of Court of all changes in his or her mailing;~ and residence, and all chazges in the name and address of his or her employei?, Within seven (7) days of such change. ~ ~ 8. That this Court reserves jurisdiction for the purpose of dete~minining the amount due fram the kespondent to ttw Petitioner, if arry, a~ reimburs~rient of past AFDC payments received by or on behalf of the beforernamed child(ren). 9. TY~at in the event the Defendant/Obligor becomes unemployed, he/she shall seek employment and he/sr,e shall cooperate with the Department of Labor and Flnployment Services of the State of Florida and malce reports to the Depart- ment of Health and Rehabilitative Services of the State o£ Florida, Child Support Fnforcement Unit, of his/her efforts to maintain employment, on a weekly basis. 10. (Applies only if box is checked) ! Th° Court finds that the Obligor has access at a reasonable rate to group health insurance. It is thereupon ordered and adjudged that said Obligor shall, in addition to all other teYms of this Order, provide health insurance for the child(ren) set forth~herein for so lang as the chiZd(ren) are dependent upon Florida law. The Obligor shall file proof of said health insurance coverage in this file and send a copy to all parties within 15 days of the date of this order. AND ORDIItID at Ft.
