, 5. That the above-named Defendant having been adjudicated the father
of the above--n3sned child(ren), the D~partm~nt of He~lth and Rehabilitative
~ervices, Bureau of Vital Statistics, Am~r~nent Unit, shall and it is hereby
ordered to am~end the above-named child's/children's birth certificate~s) to
show the above-named father's name.
6. Ttzat pursuant to Section 443.051, Florida Statutes (1985) a~d
Section 462(e) of the Title N-A of the Sacial Security Act tt~at the Department
of Labor arxi ~~ployment Security shall deduct and ~rithhold from the Uc~enployment ~
Ccxnpc~nsat ion othei~aise payable to the Defenda~nt SOx of tt~ Unei~loyment
Compensation or the amount of child support as orderd above Whichever equals
che kreater amount but does not exceed the court order~ support amounC.
7. Additionally, it is further ordered that the Respo~clent/Payor
shall promptly notify the Clerk of Court of all changes in his or her mailing
and residence, and all cha,zges in the r~ame and address of his or her employer
~.tithin seven (7) days of such change.
8. lhat this Court reserves jurisdiction for the purpose of
determinining ~he amount due from t]'ye kespos~dent to thp Petitianer, if arry, as
reimbursement of past AF'DC payments received by or on behelf of the before-r~ned
child(ren).
9. That in the event the Defendant/Obligor becaaes ~loyed, h~/she
shall seek employment and he/she shall cooperate ~tith the Department of Labor
and Fmployment Services of the State of Florida aryd make reports to the Depart-
ment of Health and Rehabilitative Services of the State of Florida, Chi2d
Support Enforcgnent Unit, of his/her efforts to maintain employment, on a
weekly basis.

'Iti~ Court finds that the Obligor has access at a ressonable rate
to group h~alth insurance. It is thereupon ordered and adjudged that said
~bligor shall, in addition to all other terms of Lhi$ Order, provide health
insurance for the child(ren) set forth herein for so long as the child(ren) are
dependent upon Florida law. The Obligor shall file proof of said health
insurance coverage in this file and send a copy to all parties vithin I5 days
of the date of this order.
