and is incorporated into and shall be deemed to amend and supplement the Mortgage, Deed of'IYust, or :~ecurity
Deed, (the "Security Instrument") of the same date given by the undersigned (the "Borrorwer") to secure
Borrower's Ac~justable Rate Note (the °Note") to NARBOK FEDERALSAVINGSAND LOAN AS.S()('IATION (the
"Lender") of the same date and co~~ering the property described in the Security Instrument and located at:

The Note contains provisions ailowing for changes in the interest rate. If the ~
interest rate increases, the Borrower's monthly psyments wilf be higher. If the
interest rate decreases, the Borrowe~'s monthly payments will be lowe~.
ADDITIONAL COYENAN7'S. In addition to the covenant and agreements made in the Security
Instrument, Borrower and Lender further co~~enant and agree as follows:
A. INTEBEST RATE AND MONTHLY PAYMENT CHANGES
The Note provides for an initial interest rate of 8• 125 Section 4 of the Note provides for change~
in the interest rate and the monthly payments, as follows:
4. INTERES'f RATE AND MONTNLY PAYMENT CHANGES
(A) Change Dates
The interest rate I will~ay may change on the firsi dayof February ,1991 , and on t}~at day ~

Bey,inning with the first Chan~e Date, my interest rate K~II be based on an "Index". The Index is thf weekly ~
average yield on United States 'IYeasury securities adjusted to a constant maturity of oNE years, os made ?
a~~ailable by the Federal Resen~e Board. The most recent Index figure a~ailabie as of 45 ~ays before each G:~ars~ ~
Date is called the "Current Index".
If the Index is no longer avaifabte, the Note Holder will choose a new Index which is based upon comparable
information. 'Ihe Note Holder will gi~~e me notice of its choice.
(C) Calculadon of Changes
~ Before each Change Date, the Note Nolder will catculate my new interest rate by adding Tw0 AND 750/1000 ~
' percentage points ( 2• 750 's~) to the Current Index. 'Ihe Note Holder will then round the result of this '
! addition to the nearest one-eighth of one percentage point (0.125~). Subject to the lirnits stated in Section 4(D) ~
! below, this rounded amount will be my new interest rate until the next Change Date. '
~ The Note Holder will then determine the amount oithe montly payment that would be su~cient to repay the
E unpaid principal that I am expected to owe at the Change Date in futl c~n the maturity date at my new interest

~ The Note Holder wil! mail or deliver to me a notice before each Change Date. The notice will advise me of: +
(i) the new interest rate on my loan as of the Change Date:
(ii) the amount of my monthly payment fo?lowing the Change Date;
(iu) Any additional mattQrs which the Note Nolder is required to disclose; and
( iv) the title and telephone number of a person who will answer any question I may have regarding the
n otice.
