, s. TTiat the above-nc~med Defendant having been adjudicated the father ;
_of the above-named child(ren), the Department nf lfealth and Rehabilitative ;
5ervices~ Bureau of Vital Statistics, Amendment Urut, shall and it is hereby ~
ordered to aaaend the above-named child's/children's birth certificate(s) to
shoW the above-nacn~d father's name.
6. Ttiat pursuant to Section 443.051, Florida Statutes (1985) and
Section 462(e) of the Title IV-A of the Socisl Security Act that the Department
of L,abor and F~nployment Security shall deduct and wiihhold fran the Un~tployment .
Compensation otheniise payable to the Defe~xiant SOX of the Unemployment
Compensation or the amount of child support as ordered above whichever equals
the greater aaaount Dut does not exceed the court ordered support amount.
7. Additionally, it is further ordered that the R~spandent/Payor
shall promptly notify the Clerk of Court of all changes in hia or her mailing
and residence, and all cha.-iges in the narne and address of his or her em~loyer ~
vithin seven (7) days of such ch~e. •
8. Tt~at Lhis Court reserves jurisdiction for Che purpose of
deterniininir~g the amount due from the kespandent to th~ Petitioner, if any~ as
reimbursement of past AFDC payments received by or on behalf of the before-r,arned
child(ren).
9. Zhat in the event the DefendanC/Obligor becomes une~nployed, helshe
shall seek eraployment s~d he/she shall cooperate wiih the Department of Labor
and Fmployment Service$ of the State of F'lorida and malce reports to the Depart-
ment of Health and Rehabilitative Services of the State of Flor~da, Child
Support Fn£orcement Unit, of his/her efforts to maintain ~nployment, on a
weekly basis.

Th4 Court finds that the Obligor has access at a reasonabl~ rate ~
to group health insurance. It is thereupon ordered and 8djudged that said ;
Obligor shall, in addition to all other tern~s of this Order, provide health ~
insurance for the child(ren) set forth herein for so long as the child(ren) are ~
dependent upon Florida lax. The Obligor shall file proof of said health :
insurance coverage in this file and send a copy to all parties within 15 days
of the date of this order.
