ORDERED AND ADJUDGED as follows: 1. That commencing on DECEMBER 3, 1982, the Responde~t shall pay $25.00 per month, plus $2.00 statutory fee, unti.l the AFI}C is reimbursed in the amount of $172.00. Said amount inc~ludes all arrears due herein. All payments - sha1Z be made in cash, money order or cashiers check. All money ordPrs and ; cashiers checks shall bear the payees name and Social Security Number and shall be made payable to the CLERK OF THE~CIRCUYT COURT, and sent to: CLERK OF CIRCUIT ~ COURT, SUFPORT DLPARTMENT, P. 0. BOX 700, FORT PIERGE, FLORIDA, 33454. Said amount shall be remitted monthly by the Clerk to the Department of Health and Rehabilitative Services, Chil.d Support Enforcement 1)nit, 1317 Winewaod Bou~evard, ~ Tallahassee, Florida, 32304. . ~ 2. That the above reimbursement paycnent shall be in addition to the ~ regular support payment hereinbefore or3ered by this ~ourt and said payment shall ~ coneinue vntil the minor.child(_ren) reaches ma~ority, dies or marries. Said regular support payment shall be sent by the Clerk of ~ircuit Court to ttie custodian of the minor child(ren) or in accordance with the most recent notice f~led by the Department. However, should the AFDC moneys be reinstated, the Petitioner •shall file a natice of such reinstatement and the Clerk shall forward - ~ regular support payments to the Department c+r in accordance with tt;e most recent notice filed by the Department. For the~time_period the custodian of the minor ~ child(ren) is receiving !~FDC moneys, the reimbursement payment as set forth above ~ shall be deferred. Upon cancellation of AFDC moneys paid to, or on behalf of, the j child{ren) the Department shall fil'e a notice an3 the Respondent shall i.mmediately - } commence paying the reimbursement payment as set forth above. The Clerk shall ; forward said payments in accordance with the most recent notice filed by tHe ~ ~ Department. i ~ 3, Additionally, it is hereby ardered pursuant to Chapter 82-I40, Laws ~ of Florida (1982), that the emplo}~er of the Respondent sha~_1 deduct the sums herein provided on a periodic basis from the income due the Respondent and remit - same, after deducting its charge for handling, to the Court Registry. Such income deduction snall not be effective until the Respondent is delinquent in two (2) child support payments and has failed..to pay ~all arrearages and past public assistance obligations. Service of this order on th~ employer by the office of Child Suppc~rt Enforcement of t~e Florida Department ~f iteal.th and Rehabilitative Services shall be prima facie evidence that~the above ca~~"*ions havP occurred. Upon receipt c~f this order, the employer shall govern hi.mself accordingl,y and ~ remit payment forthwith.
