We are pleased to submit the following proposal for your consideration on the above referenced premises as follows.
We agree to provide all labor, permits, material, tools, equipment, and the proper insurance in excess of two million
dollars.
WORK SCOPE: Sloped Roof Area
1. Remove existing roof to the plywood deck. Discard all debris legally off site,
2. Re -fasten the existing plywood to meet today's building code requirement.
3. Furnish and install Tri -built Sand Self Adhering Underlayment in accordance with local building code.
4. Furnish and install 2.5 LB lead stack flashing on all plumbing vents and new zipper boot for electrical pipe.
5. Furnish and install 26 -gauge galvanized mill finish valley metal to code.
6. Furnish and install 26 -gauge painted (brown or white) galvanized eave and gable metal to code.
7. Furnish and install Owens Corning Tru -Definition Dimensional Shingle Roof S stem, including all standard
and trim, 30 lin. ft. of ridge vent to code with manufacturer' standard colors. , �L_25

EXCLUSIONS
The Roof Authority, Inc. ("TRA") specifically excludes the following, disclaims all liability for claims related to the same,
and notifies the owner that these items must be addressed by others and will be at owner's expense:
■ Gutters & Downspouts.
• Concrete, lightweight, steel, wood, or any other form of deck replacement, other than that specifically noted herein.
■ Provisions regarding wind mitigation requirements, if required.
• Mechanical / Electrical / Plumbing / AC Stands/ Solar panels & Satellite dishes.
• Removal of more than one roof, or siding, unless specifically stated herein.
• Tapered roof insulation and/or ponding water.
• Damage to person or property caused by mold, mildew, fungi, spores, algae, microscopic organisms, hazardous
chemicals, biological agents or allergens.
■ If, the subject property is exposed to windstorms or hurricanes in excess of the designed wind speed, all warranties
provided by TRA, if any, shall be deemed null and void. In addition, if Buyer fails to strictly adhere to the payment
terms contained in the contract, the warranty shall also be deemed null and void.
• Leaks or other damage caused by:
o Natural disasters including, without limitation, floods, lightning, hurricanes, tropical storms, hail,
windstorms, earthquakes, and/or tornadoes, provided that the damage is not solely the result of the failure of
the roof system to meet the designed wind speed.
o Structural failures such as cracks in decks, driveways, walls, partitions, foundations, windows, stoppage of
roof drains or gutters, etc.
o Changes in original principal usage to which building is put unless approved in advance in writing.
o Erection or construction of any additional installation on or through the roofing felt after date of
completion.
o Roof or flashing repairs by others; painting or coating without approval.
o Acts of God, strikes, riots, war, civil disturbances, fire, vandalism or other damage beyond TRA's control.
o Dry rot, termites, rodents, or other pests.
o Penetration of the roofing from beneath by rising nails.
o Failure of Buyer to maintain the roof system and/or damage caused by foot traffic.
o Latent manufacturing defects of any roofing or flashing materials that materially affect their performance.
■ Damage to the building or its contents, roof insulation, roof deck or other base over which roofing felt is applied.

UNIT PRICES
■ Replacement of plywood decking (if needed) - $3.00 per square foot (Minimum % sheet per location)
■ Additional layer of roof removal $1.00 per sq. ft.
■ Additional work (if needed), beyond the scope of our contract will be charged at material cost plus 25% and labor rate of
$95.00 per man-hour.
The work will be accomplished and guaranteed in accordance with TRA's Standard General Conditions and Limited Warranty,
which are made a part of this Proposal and Agreement, and incorporated herein by reference. YOUR SIGNATURE BELOW
ACKNOWLEDGES RECEIPT AND APPROVAL OF THESE DOCUMENTS.
All labor and material necessary to perform the above work will be furnished for the sum Eight Thousand Eight Hundred
Seventy -Five Dollars ($8,875.00) (IF TAX EXEMPT, CERTIFICATE REQUIRED UPON EXECUTION OF THIS PROPOSAL
AND AGREEMENT), payable at TRA's office in Fort Pierce, Florida, in full, upon completion of the work. If the entire job is not
completed within any calendar month, progress payment for all labor and/or materials on the job by the last day of each month are
due no later than the tenth (10") day of the succeeding month until the time final payment, upon completion, is due.
Twenty Five (25) Percent of the contract amount due upon acceptance of the owner. Fifty (50) percent when the roof has
been removed and dried in. Balance upon completion.
Additional 3%, added if waving with credit card.
This is a confidential agreentent. Please do not duplicate or share infornialion contained herein.
Prior to initiation of any action, the parties agree to participate, in good faith, in a minimum half-day mediation with a mediator
either chosen by agreement between the parties or as assigned by the American Arbitration Association. In the event the dispute
is not resolved by mediation, any claim or controversy arising out of or relating to this Agreement or breach thereof, or to any
action by an employee or agent of TRA, shall be settled by arbitration in accordance with the Federal Arbitration Act and
Construction Industry Arbitration Rules of the American Arbitration Association. However, the arbitrator shall be required to
issue written findings of fact and conclusions of law. Moreover, the parties shall split the cost of keeping a written record of all
proceedings. The judgment of the Arbitrator may be entered in any court having jurisdiction thereof and shall be final and
binding on both parties, except for errors of law, which shall be appealable. The parties agree that all expenses of arbitration or
mediation will be shared equally or as awarded by the arbitrator. The successful party shall also be entitled to attorney fees and
all costs of litigation.
Due to the extreme price volatility regarding petroleum products, the price quoted in this proposal is valid only for
orders placed and paid within the next 30 days. If there is an increase in the price paid by The Roof Authority, Inc. for
asphalt, polyisocyanurate, steel or other materials, including transportation charges, the amount of this
proposal/contract shall be similarly increased to reflect the increased costs to obtain the materials.

SCOPE:
This Evaluation Report is issued under Rule 61G20-3 and the applicable rules and regulations governing the use of
construction materials in the State of Florida. The documentation submitted has been reviewed by Robert Nieminen,
P.E. for use of the product under the Florida Building Code and Florida Building Code, Residential Volume. The
products described herein have been evaluated for compliance with the 6th Edition (2017) Florida Building Code
sections noted herein.
DESCRIPTION: TRI -BUILT Roof Underlayments
LABELING: Labeling shall be in accordance with the requirements the Accredited Quality Assurance Agency noted
herein.
CONTINUED COMPLIANCE: This Evaluation Report is valid until such time as the named product(s) changes, the referenced
Quality Assurance documentation changes, or provisions of the Code that relate to the product change. Acceptance of
this Evaluation Report by the named client constitutes agreement to notify Robert Nieminen, P.E. if the product
changes or the referenced Quality Assurance documentation changes. Trinity) ERD requires a complete review of this
Evaluation Report relative to updated Code requirements with each Code Cycle.
ADVERTISEMENT: The Evaluation Report number preceded by the words "Trinity IERD Evaluated" may be displayed in
advertising literature. If any portion of the Evaluation Report is displayed, then it shall be done in its entirety.

4. PRODUCT DESCRIPTION:
4.1 Tri -Built Sand -R SA Shingle Underlayment is a self -adhering, glass -mat reinforced, sand -surfaced, SBS
modified bitumen roof underlayment, with a 36 -inch sheet width; meets ASTM D1970.
4.2 Tri -Built S/A HT TU Underlayment is a self -adhering, non -woven polyester fabric surfaced, SBS modified
bitumen roof underlayment; meets ASTM D1970 and FRSA/TRI April 2012.
4.3 Tri -Built Smooth HT S/A Underlayment is a self -adhering, glass -mat reinforced, film -surfaced, SBS
modified bitumen roof underlayment; meets ASTM D1970.
5. LIMITATIONS:
5.1 This is a building code evaluation. Neither TrinityJERD nor Robert Nieminen, P.E. are, in any way, the
Designer of Record for any project on which this Evaluation Report, or previous versions thereof, is/was

5.4 TRI -BUILT Roof Underlayments may be used with any prepared roof cover where the product is
specifically referenced within FBC approval documents. If not listed, a request may be made to the
Authority Having Jurisdiction for approval based on this evaluation combined with supporting data for the
prepared roof covering.

5.6 Allowable Substrates:
5.6.1 Direct -Bond to Deck:

Underlayment applied to:

While not required over plywood, OSB or Southern Yellow Pine substrates, TRI -BUILT Materials Group, LLC
recommends priming the deck if the final roof cover is not slated for installation within 24 hours. Note:
TRI -BUILT Materials Group, LLC requires tongue -and -groove board decking be covered with plywood or
OSB prior to installation of the self -adhering underlayment.
5.6.2 Bond -to -Insulation:

minimum requirements of the prepared roof covering manufacturer's Product Approval. For installations
under foam -on tile systems (Tri -Built S/A HT TU Underlayment only), insulation attachment shall be
designed by a qualified design professional and installed based on testing of the insulation/underlayment
system in accordance with FM 4474, Appendix D or TAS 114, Appendix J.

5.6.3 Bond to Mechanically Attached Base Layer:

Underlayment applied to:
➢ ASTM D226, Type I or II felt.
For installations under mechanically attached prepared roof coverings, base layer shall be attached per
minimum codified requirements. For installations under foam -on tile systems (Tri -Built S/A HT TU
Underlayment only), base layer shall be attached per minimum requirements of FRSA/TRI April 2012 (04-
12).
5.6.4 Wind Resistance for Underlayrment Systems in Foam -On Tile ApplIcation5: FRSA/TRI April 2012 (04-12)
does not address wind uplift resistance of all underlayment systems beneath foam -on tile systems, where
the underlayment forms part of the load -path. The following wind uplift limitations apply to direct -deck
underlayment systems that are not addressed in FRSA/TRI April 2012 (04-12) and are used in foam -on tile
applications. Maximum Design Pressure is the result of testing for wind load resistance based on
allowable wind loads, and reflects the ultimate passing pressure divided by 2 (the 2 to 1 margin of safety
per FBC 1504.9 has already been applied). Refer to FRSA/TRI April 2012 (04-12), Appendix A, Table 1A or
FBC 1609 for determination of design wind pressures.
#1 Maximum Design Pressure = -45.0 sf:
Deck: Min. 15/32 -inch plywood to meet project requirements to satisfaction of Authority
Having Jurisdiction.

5.6.4.1 For mechanically attached Base Sheet, the maximum design pressure for the selected assembly shall meet
or exceed that required under FRSA/TRI April 2012 (04-12), Appendix A, Table 1A.
Alternatively, the maximum design pressure for the selected assembly shall meet or exceed the Zone 1
design pressure determined in accordance with FBC 1609. In this case, Zones 2 and 3 shall employ an
attachment density designed by a qualified design professional to resist the elevated pressure criteria.
Commonly used methods are ANSI/SPRI WD3, FM Loss Prevention Data Sheet 1-29 and Roofing
Application Standard RAS 117. Assemblies marked with an asterisk* carry the limitations set forth in
Section 2.2.10.1 of FM Loss Prevention Data Sheet 1-29 (January 2016) for Zone 2/3 enhancements.
5.7 Exposure Limitations:

exposed for longer than 30 -days after installation

When loading roof tiles on the underlayment in direct -deck tile assemblies, the maximum roof slope shall
be as follows. These slope limitations can only be exceeded by using battens during loading of the roof
tiles.

6. INSTALLATION:
6.1 TRI -BUILT Roof Underlayments shall be installed in accordance with TRI -BUILT Materials Group, LLC
published installation requirements subject to the Limitations set forth in Section 5 herein and the
specifics noted below.
6.2 Re -fasten any loose decking panels, and check for protruding nail heads. Sweep the substrate thoroughly
to remove any dust and debris prior to application, and prime the substrate (if applicable).
6.3 Tri -Built Sand -R SA Shingle Underlayment, Tri -Built SIA HT TU Underlayment or Tri -Built Smooth HT SIA
Underlayment:
6.3.1 Shall be installed in compliance with the requirements for ASTM D1970 underlayment in FBC Table
1507.1.1 for the type of prepared roof covering to be installed.
6.3.2 Non -Tile applications:
While priming is optional, TRI -BUILT Materials Group, LLC recommends priming the deck if the final roof
cover is not slated for installation within 24 hours.
Apply sheet parallel to the roof edge. Roll out approximately 10 ft of membrane and peel back the first 3
ft of release film.
Adhere the exposed part to the substrate and unroll the remaining membrane as far as possible.
Once the entire length of membrane is in place, peel -off the release film diagonally while holding the
membrane tight. Firmly roll, broom or hand -press the membrane into place to achieve a bond.
Horizontal seams should be minimum 3 -inches, configured to shed water. Vertical seams should be 6 -
inches and staggered not less than 2 -ft from vertical seams in the course below, sealed in accordance with
TRI -BUILT Materials Group, LLC requirements.
When installing at slopes above 8:12, TRI -BUILT Materials Group, LLC recommends back -nailing in the
overlap area at the top of the sheet at 12 -inch o.c.
For Valleys and Ridges: Cut underlayment into 4 to 6 foot lengths. Peel the release paper and center sheet
over valley or ridge. Drape and press sheet into place, working from the center of the valley or ridge in
each direction. For valleys, apply the sheet starting at the lowest point and work upward.
6.3.3 Tile Applications (Tri -Built SIA HT TU Underlayment only):
Reference is made to FRSA/TRI April 2012 (04-12) Installation Manual and Table 1 herein, using the
instructions noted above as a guideline.
For foam -on tile applications, reference is made to Section 5.6.4 herein for wind resistance limitations that
fall outside the scope of FRSA/TRI April 2012 (04-12).
Tile shall be loaded and staged in a manner that prevents tile slippage and/or damage to the
underlayment. See Table 2 herein, and TRI -BUILT Materials Group, LLC published requirements for tile
staging.
7. BUILDING PERMIT REQUIREMENTS:
As required by the Building Official or Authority Having Jurisdiction in order to properly evaluate the installation of
this product.

5.4 Wind Classification:
5.4.1 All Owens Corning asphalt shingles noted herein are Classified in accordance with FBC Tables 1507.2.7.1
and R905.2.6.1 to ASTM D3161, Class F and/or ASTM D7158, Class H, indicating the shingles are acceptable
for use in all wind zones up to Vasd = 150 mph (V,,it = 194 mph). Refer to Section 6 for installation
requirements to meet this wind rating.
5.4.2 All Owens Corning hip & ridge shingles, Starter Strip Shingle and Starter Strip Plus noted herein are
Classified in accordance with FBC Tables 1507.2.7.1 and R905.2.6.1 to ASTM D3161, Class F, indicating the
shingles are acceptable for use in all wind zones up to Vasd = 150 mph (V,,it = 194 mph). Refer to Section 6 for
installation requirements to meet this wind rating.
5.4.3 Classification by ASTM D7158 applies to exposure category B or C, as defined in FBC 1609.4.3, and a mean
roof height of 60 feet or less. Calculations by a qualified design professional are required for conditions
outside these limitations. Contact the shingle manufacturer for data specific to each shingle.
5.4.4 Refer to Owens Corning published information on wind resistance and installation limitations.
5.5 All products in the roof assembly shall have quality assurance audit in accordance with F.A.C. Rule 61G20-3.
6. INSTALLATION:
6.1 Underlayment:
6.1.1 Underlayment shall be acceptable to Owens Corning and shall hold current Florida Statewide Product
Approval, or be Locally Approved per Rule 61G20-3, per FBC 1507.2.3, 1507.2.4 or R905.2.3.
6.2 Asphalt Shingles:
6.2.1 Installation of asphalt shingles shall comply with the Owens Corning current published instructions, using
minimum four (4) nails per shingle in accordance with FBC 1507.2.7 or R905.2.6, with the following
exceptions:
➢ Berkshire shingles require minimum five (5) nails per shingle.
➢ WeatherGuarde HP shingles require minimum six (6) nails per shingle.
➢ Devonshire'" shingles require minimum six (6) nails per shingle.
➢ Starter Strip Shingle and Starter Strip Plus require minimum five (5) nails per strip.
Refer to Owens Corning published information on wind resistance and installation limitations.
6.2.2 Fasteners shall be in accordance with the manufacturer's published requirements, but not less than FBC
1507.2.6 or R905.2.5. Staples are not permitted.
6.2.4 Where the roof slope exceeds 21 units vertical in 12 units horizontal, special methods of fastening are
required. See figures below for details.

7.1 Labeling shall be in accordance with the requirements the Accredited Quality Assurance Agency noted
herein.
7.2 Asphalt shingle wrappers shall indicate compliance with one of the required classifications detailed in FBC
Table 1507.2.7.1 / R905.2.6.1.
8. BUILDING PERMIT REQUIREMENTS:
As required by the Building Official or Authority Having Jurisdiction in order to properly evaluate the installation of
this product.
9. MANUFACTURING PLANTS:
Contact the named QA entity for information on which plants produce products covered by Florida Rule 9N-3 QA
requirements.
