This Assignment of Insurtince Benefits("Agreement")is between COMPANY(Advanced Metal Roofing,Inc,DBA Bril-
liant Roofing) (collectively,"Parties"and individually"Party").Owner agrees to the terms and conditions below and`euthorizes
Company to enter Owner's property,furnish materials,supply equipment;and perform labor in connection with the above-referenced
Claim.
ASSIGNMENT OF INSURANCE BENEFITS:Owner assigns all Insurance rights,benefits,proceeds,claims,and causes of
action under any applicable insurance policies (collectively, "Benefits")to Company including,without limitation, claims
for bad faith,attorney's fees,and costs pursuant to Florida Statutes,for services rendered or to be rendered by
Company.Owner Intends for all Benefits related to or for services tendered by,Company, related to Company's labor,
materials,and related costs("Services")to be assigned solely and exclusively to Company.In this regard,Owner waives
all privacy rights.Owner agrees that Company may freely sell,convey,transfer,or assign this Agreement.Owner makes
this assignment in consideration for Company's agreement to perform the Services, supply materials, and perform its
obligations under this Agreement.This assignment In no way obligates Company to perform services unless and until
Company redeives cleared,funds sufficient to perform repairs at the Loss Address,to be determined In the Company's
sole and absolute discretion. Owner.directs Owner's Insurance carder(s)("Carrier")to release all information requested
by Company,Its representatives,and/or Its attorney to obtain actual benefits to be paid by Carrier for services rendered
or to be rendered.If Owner refuses to cooperate with the Carrier,refuses to endorse and deliver to Company any check
from Carrier,falls to comply with the applicable Insurance policies,or otherwise interferes with Company providing
Services or being paid as provided herein,Owner will be in default hereof and,if applicable,late charges of 1.5%
monthly will be charged on the unpaid balance.Company is entitled to reimbursement from Owner for reasonable
attorney's fees,costs of enforcing this Agreement as well as Company's administrative time spent enforcing this

damage to Owner's roof.Owner covenants that Owner has not previously assigned the Benefits and that if Owner did
previously assign the Benefits,that Owner is In default hereof.Owner acknowledges and understands that Owner has no
right of rescission under this Agreement.
DIRECTION TO PAY:Owner authorizes and unequivocally and irrevocably Instructs direct payment of any Benefits for
Services paid jointly to Company and Owner but sent only to Company.Owner agrees that any portion of the Services
not covered by insurance is Owner's responsibility. Owner represents that all necessary parties have executed this
Agreement.
ENTIRE AGREEMENT:Owner agrees that this contract is the complete and total agreement between Owner and
Company.If any provision is held invalid or unenforceable by a court of competent jurisdiction,the Parties agree that the
remaining provisions will remain in full force and effect.The Parties waive the right to claim that they were induced to
enter Into this Agreement by anything not expressly set forth herein.The Parties acknowledge that they:(i)had the
opportunity to consult with an attorney of their choosing prior to signing this Agreement or waive their right to do so;
and(11)fully understand this Agreement.
ACTS OF GOD:Company's performance Is excused in the event of fire,explosion,flood,severe weather,accident, .
governmental act, embargo, shortages of materials, computer system failure, war or military action, riot, or any other
event,beyond the control of Company or which materially affects the economic basis of the bargain ("force majeure").
Company's performance in the event of a force majeure will be excused for as long as the cause continues,without
liability.If the force majeure continues unabated for ninety(90)consecutive days.Company may,in its sole and absolute
discretion,cancel this Agreement without liability.

STOP WORKIIIOLD HARMLESS.: If Owner does not allow Company to perform its recommended procedures, Owner
hereby releases,holds Company harmless,and Indemnifies Company against all claims or actions that may result from
such-same.Owner-also indemnifies Company from and against all allegations,claims,actions,suits,demands,
damages,liabilities,obligations,losses,settlements,judgments,costs and expenses(including without limitation
attorneys'fees and costs)which arise out of,relate to or result from any act or omission of Owner.Company will make all
reasonable efforts to protect the property from any further damages.However,Company is not liable for wear and tear to
driveways,parking lots,walkways,lawns,shrubs,trees,or other damage to vegetation,fixtures,or Improvements caused
by truck, equipment, m4terials,workers, or debris. Owner further agrees to promptly notify Company if Owner receives
any communication fro y third f e corning Company.

SCOPE:
This NOA'is being issued under the applicable rules and regulations governing the use of construction materials.The
documentation submitted has been reviewed and accepted by Miami Dade County PERA-Product Control Section
to be used in Miami Dade County and other areas where allowed by the Authority Having Jurisdiction(AHJ).
This NOA shall not be valid after the expiration date stated below.The Miami-Dade County Product Control Section
(In Miami Dade County)and/or the AHJ(in areas other than Miami,Dade County)reserve the right to have this
product,or material tested for quality assurance purposes.If this product or material fails to perform in the accepted
manner,the manufacturer will incur the expense of such testing and the AHJ may immediately revoke,modify,or
suspend the use of such product or material within their jurisdiction. PERA reserves the right to revoke this
acceptance,if it is determined by Miami-Dade County Product Control Section that this product or material fails to
meet the requirements of the applicable building code.
This product is approved as described herein,and has been designed to comply with the Florida Building Code
including the High Velocity Hurricane Zone of the Florida Building Code.

LABELING:Each unit shall bear a permanent label with the manufacturer's name or logo,city,state and following
statement: "Miami-Dade County Product Control Approved",unless otherwise noted herein.
RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been no
change in the applicable building code negatively affecting the performance of this product. .
TERMINATION of this NOA will occur after the expiration date or if there has been a revision or change in the
materials,use,and/or manufacture of the,product or process.Misuse of this NOA as an endorsement of any product,
for sales,advertising or any other purposes shall automatically terminate this NOA.Failure to comply with any
section of this NOA shall be cause for termination and removal of NOA,
ADVERTISEMENT: The NOA number preceded by the words Miami-Dade County,Florida, and followed by the
expiration date may be displayed in advertising literature. If any portion of the NOA is displayed, then it shall be
done in its entirety.

3. LIMITATIONS
3.1 Fire classification is not part of this acceptance.
3.2 For mortar or adhesive set tile applications, a static field uplift test shall be performed in
accordance with TAS 106.
3.3 Applicant shall retain the services of a Miami-Dade County Certified Laboratory to perform
quarterly test in accordance with TAS 112, appendix 'A'. Such testing shall be submitted to
the Building Code Compliance Office for review.
3.4 Minimum underlayments shall be in compliance with the applicable Roofing Applications
Standards listed section 4.1 herein.
3.5 30/90 hot mopped underlayment applications may be installed perpendicular to the roof slope
unless stated otherwise by the underlayment material manufacturers published literature.
3.6 This acceptance is for wood deck applications. Minimum deck requirements shall be in
compliance with the applicable Building Code.
4.' INSTALLATION
4.1 Barcelona 900 Concrete Roof Tile and its components shall be installed in strict compliance
with Roofing Application Standard RAS 118,RAS 119,and RAS 120.

8. Install a few fasteners at the top,near the rake,and rollout the sheet to a manageable length.
9. Pull,straighten and align the sheet so that any wrinkles are eliminated and the sheet is even with the cave edge.
10. EasyLay shall be fastened with approved roofing,nails and tin caps or approved cap nails,driven by hand or
pneumatically,spaced-6"o.c.at all laps in the center of the seam area,and two staggered rows fastened 12"
o.o.in the field of the sheet.
11. When installing nails,apply so that the head of the nail is flush with the EasyLay surface,without cutting into
the EasyLay surface.
12. Fastening shall be done from the top to avoid walking or kneeling on unsecured sheet.
13. Continue on to the end of the substrate and fasten down.
14. Align the next roll over the preceding sheet so as to form a minimum 4"overlap.
15. Then install the just placed sheet,per instructions above.
16. The bottom of the second course of EasyLay shall lay on top of the first course so that any water will flow over
the top of EasyLay.
17. Apply subsequent sheets in the same manner,with 4"side laps and 6"end laps over the preceding sheets.
18. When the top course is reached,lap about 6"of EasyLay over the ridge.

19. Install EasyLay a minimum of 6"up any vertical surfaces.
20. Stagger the end laps a minimum of 18"from the preceding course.
21. After installing EasyLay on the field of the roof,install drip edge at the eaves(if used).
22. When applying EasyLay in the valley,start at the low point and work to the high point,rolling the membrane
from the center outward in each direction. Make sure there are no rips or large wrinkles in the EasyLay.
23. Before applying horizontal sections of EasyLay,install a vertical length of EasyLay down the center of the
valley.

7. All approved substrates are to be clean,dry and free of any loose debris or moisture prior to the application of
the Tarco self adhering underlayments. Refer applicable building codes prior to installation to verify
acceptable substrates.
8. Prime all metal collars,flashing,valleys,liner,drip edge and concrete deck substrate with ASTM D 41 primer,
water based acrylic or water based polymer modified primer.
9. Contractor may out the underlayments into sections for workability and allow to relax prior to application.
10. Place the underlayments over metal drip edge in accordance with RAS I 1 1.
11. Install the first course of underlayment parallel to the eave edge.
12. Apply the underlayment,working from the center of the material continuously to the ends of the sheet(half of
the length of the sheet is the center);taking care to avoid wrinkles and ridges.
13. Remove the underlayments release film rapidly in a continuous fashion. Ensure the bottom adhesive side of
the membrane does not adhere to its self. In the event this transpires,separate the two layers immediately.
After some time,it may become impossible to do so without damaging the material.
14. It is recommended that end laps be staggered a minimum of 18" from the preceding course.
15. Underlayments are to be back nailed along the head tap.The nails shall be, I 1 gauge 1 i/a'approved ring shank
type applied with a minimum of a 1$/s"approved tin cap or approved capnail as required per the High Velocity
Hurricane Zone(HVHZ)section of the FBC,at a rminimum rate of 9"o.c. The head lap of the preceding layer
of underlayment is to cover the area being back nailed.
16. Roll or broom the entire membrane surface paying special attention to all overlap areas"side laps,end laps,T-
joints"to ensure adhesion with acceptable substrates. A minimum of a 28 lb weighted roller may be used for
steep slope applications. The use of a soft bristled push broom is acceptable on steeper slopes. The above
mentioned procedures are necessary in order to apply uniform pressure and allow for contact of the
membranes.
17. Apply t/s"thick uniform layer of SBS trowel grade modified bitumen asphalt adhesive/flashing cement
throughout the contact area of the d"granule over granule and fabric over fabric end laps. Once the
aforementioned procedure has been completed,the membrane must then be hand rolled in place in order to
ensure contact of membrane and achieve'a minimum of 1/8s11"asphaltic bleed out in designated area.
18. Tarco self adhering underlayments shall be applied to protrusions,slope changes,valleys,curbs,and other roof
top penetrations before any other sections of the roof.
19. When applying underlayments in the valley,start at the low point and work to the high point,rolling the
membrane from the center outward in each direction.

20. For ridge applications,center the underlayment and roll from the center outward in both directions.
21. Flash vent pipes,stacks,chimneys and penetrations in compliance with Roof Assembly current Product
Control Notice of Acceptance and applicable Building Code.
22. All protrusions or drains shall be initially taped with a 6"piece of like kind membrane. The flashing tape shall
be pressed in place and formed around the protrusion to ensure a tight fit. A second layer of like kind
membrane shall be applied over the flashing detail.
23. Vertical strapping of Tarco self adhering membranes is.acceptable.
GENERAL LiNITATIONS--TARCO SELF-ADHERED UNDERLAYMENTS:
1. Fire classification is not part of this acceptance;refer to a current Approved Roofing Materials Directory for fire
ratings of this product.
2. This acceptance is for prepared roofing applications. Minimum deck requirements shall be in compliance with
applicable building code.
3. Tarco self adhering underlayments shall be applied only when material interface temperatures are 400 F and
rising.
4. Tarco self-adhered underlayinents shall not be installed when any form of moisture such as water,dew,rain,etc.
is present on the substrate.
5. Tarco self-adhered underlayments are to be applied to a smooth,clean and diy surface,with the deck free from
irregularities.
6. Ensure roof has positive drainage prior to installation of Tarco self-adhered underlayments.
7. Tarco self-adhered underlayments shall not be applied over a pre-existing roof membrane or system.
8. After installation of Tarco self-adhered membranes,waita, minimum of 48 hours before roof loading of tiles.
9. Care should be taken during the loading:procedure to keep foot traffic to a minimum and avoid dropping the
roof covering directly on the underlayment.
10'. All tiles shall be staged(two tiles perpendicular to slope,four tiles on top parallel to slope)as per
manufacturer's requirements,not to exceed 6-high,to the standard maximum roof pitch of 6:12 for flat tiles
and 6:12 for lugged tiles(See Tile Staging Method diagram below).
At roof slopes greater than the above limitations,the use of loading battens or toe boards are required to load the
roof tile.

11. The manufacturer reserves the right to change the tile staging method at any time as well as the number of tiles
stacked, not to exceed the preceding maximum number of tiles limitation.
12. EasyLay,P5200"u,MS300,SS400,and NR5001shall not be lett exposed as a temporary roof for longer than
30 days after application. PS200Hr shall not be left exposed as a temporary roof for longer than 120 days after
application. Fast90 shall not be left exposed as a temporary roof for longer than 180 days after application. The
manufacturer reserves the right to change product exposure period at any time;not to exceed the preceding
maximum time limitations.
13. NR50e is not recommended for extreme high temperature environments such as under copper or zinc metal
roofing.
14, Refer to prepared roofing system Product Control Notice of Acceptance for listed approval of Tarco
underlayments with specific prepared roofings ducts. Tarco self-adhered-underlayments may be used with
any approved roof coverings Notice of Acceptance listing Tarco self adhering underlayments or the ASTM
standard which the self adhering underlayments meet as a component part of an assembly in the Notice of
Acceptance.
If Tarco self-adhered underlayments or the ASTM standards are not listed,a request may be made to the
Authority Having Jurisdiction(AHJ)or the Miami-Dade County Product Control Department for approval
provided that appropriate documentation is provided to detail compatibility of the product,wind uplift
resistance,and fire testing results.
15. All products lised herein shall have a quality assurance audit in accordance with the Florida Building Code and
Rule 61G20-3 of the Florida Administrative Code,
MANUFACTURER'S CONSIDERATIONS:
1. When using Fast90 and PS200"in the roofing applications,use of loading battens or toe boards on roof slopes
greater than 6:12 and higher is recommended.When using EasyMop SBS in the roofing applications,use of
loading battens or toe boards on roof slopes greater than 5:12 for.Flat Tiles and 6:12 for Lugged Tiles and higher
is recommended.
2. Code body requirements supersede manufacturer's recommendations and installation guidelines.
BUILDING PERMIT REQUIREMENTS:
Application for building permit shall be accompanied by copies of the following:
1. This Notice of Acceptance.
2. ' Any other documents required by the Building Official or applicable building code in order to properly evaluate
the installation of these'materials.

5.6.4 Wind Resistance for Underlavment Systems in Adhesive-set Tile Applications FRSA/TRI April 2012(04-12)does
not address wind uplift resistance of all underlayment systems beneath adhesive-set tile systems, where the
underlayment forms part of the load-path. The fallowing wind uplift limitations apply to underlayment systems
that are not addressed in FRSA/TRI April 2012(04-12)and are used In adhesive-set tile applications. Maximum
Design Pressure is the result of testing for wind load resistance based on allowable wind loads,and reflects the
ultimate passing pressure divided by 2(the 2 to 1 margin of safety per FBC 1504.9 has already been applied).Refer

When loading roof tiles on the underlayment in direct-deck the assemblies,the maximum roof slope shall be as
follows. These slope limitations can only be exceeded by Using battens during loading of the roof tiles.

6. INSTALLATION:
6.1 Boral Roof Underlayments shall be installed In accordance with Boial Roofing published Installation Instructions
subject to the Limitations set forth in Section 5 herein and the specifics noted below.
6.2 Re-fasten any loose decking panels, and check for protruding nail heads. Sweep the substrate thoroughly to
remove any dust and debris prior to application,and prime the substrate(if applicable).
6.3 Install self-adhering underlayment when ambient and surface temperatures are minimum 40°F and rising.

6.4.1 Citadel Pius is limited to use as a base or mid-layer in multi-ply underlayment systems beneath Boral TIIeSeal 501'T,
Boral TileSeall or GatorSeal.
6.4.2 Slope limitations are those associated with the top-layer underlayment.
6.4.3 Direct to Deck:
Cut the membrane into manageable lengths,typically 10 to 12 ft.Align the membrane parallel to the roof edge,
extending over by%-inch. Fold.the membrane away from the edge onto itself. Remove the release sheet. Place
the membrane with the exposed rubberized asphalt onto the deck,pressing firmly into place. Roll into place with
a weighted roller.
Fasten the top edge of the sheet to the deck with corrosion resistant,roofing nails, nails & tin-tags or 1-inch
diameter plastic or steel cap nails spaced maximum 12-inch o.c. Overlap successive courses the entire width of the
black-selvedge area,minimum 2 Inches(horizontal lap)and minimum 6"end(vertical)laps.
install final underlayment layer atop Citadel Plus per 'direct-to-deck' instructions for Boral TileSeal 50HT, Boral

6.4.4 To Base Sheet:

Install optional Citadel Plus as a mid-ply In accordance with Boral Roofing published Installation Instructions and

Install final underlayment layer atop Citadel Plus per 'direct-to-deck' instructions for Boral TileSeal 50HT, Boral
TileSealHT or GatorSeal.
Note:For adhesive-set the systems,if there is a base ply or mid-ply of Citadel Plus in the system,the project design
pressure requirements may not exceed-120 psf.

6.5.1 Shall be installed in compliance with the requirements for ASTM D1970 underlayment in FBC Table 1507.1.1 for
the type of prepared roof covering to be installed.
6.5.2 Do not use Boral TileSeal 501 on roof pitches less than 2:12.
6.5.3 Non Tile Applications.Direct to Deck:
Cut the membrane Into manageable lengths,typically 10 to 12 ft.Align the membrane parallel to the roof edge,
