This Evaluation Report is issued under Rule 61620-3 and the applicable rules and regulations governing the use of
construction materials in the State of Florida. The documentation submitted has been reviewed by Robert Nieminen,

4.1.4 Duration®, TruDefinition® Duration®, Duration® Premium Cool, TruDefinition® Duration Designer Color
Collection, TruDefinition Oakridge , Oakridge and WeatherGuard HP are fiberglass reinforced, laminated
asphalt roof shingles.
4.2 Hip&Ridge Shingles:
4.2.1 Berkshire Hip & Ridge Shingles, High Ridge, WeatherGuard HP Hip & Ridge Shingles, Pro Edge Hip & Ridge
Shingles and DuraRidg6l Hip&Ridge Shingles are fiberglass reinforced,hip and ridge asphalt roof shingles.
4.3 Accessory Starter Strips:
4.3.1 Starter Strip Shingle,Starter Strip Plus and Starter Shingle Roll are starter strips for asphalt roof shingles.
5. LIMITATIONS:
5.1 This is a building code evaluation. Neither TrinityJERD nor Robert Nieminen, P.E. are, in any way, the
Designer of Record for any project on which this Evaluation Report,or previous versions thereof,is/was used
for permitting or design guidance unless retained specifically for that purpose.
5.2 This Evaluation Report is not for use in FBC HVHZ jurisdictions.
5.3 Fire Classification is not part of this Evaluation Report;refer to current Approved Roofing Materials Directory
for fire ratings of this product.

5.4 Wind Classification:
5.4.1 All Owens Corning asphalt shingles noted herein are Classified in accordance with FBC Tables 1507.2.7.1
and R905.2.6.1 to ASTM D3161,Class F and/or ASTM D7158,Class H, indicating the shingles are acceptable
for use in all wind zones up to Vasa = 150 mph (V„tt = 194 mph). Refer to Section 6 for installation
requirements to meet this wind rating.
5.4.2 All Owens Corning hip & ridge shingles, Starter Strip Shingle and Starter Strip Plus noted herein are
Classified in accordance with FBC Tables 1507.2.7.1 and R905.2.6.1 to ASTM D3161, Class F, indicating the
shingles are acceptable for use in all wind zones up to Vasd=150 mph(V„it=194 mph). Refer to Section 6 for
installation requirements-to meet this wind rating.
5.4.3 Classification by ASTM D7158 applies to exposure category B or C,as defined in FBC 1609.4.3, and a mean
roof height of 60 feet or less. Calculations by a qualified design professional are required for conditions
outside these limitations. Contact the shingle manufacturer for data specific to each shingle.
5.4.4 Refer to Owens Corning published information on wind resistance and installation limitations.
5.5 All products in the roof assembly shall have quality assurance audit in accordance with F.A.C.Rule 61G20-3.
6. INSTALLATION:
6.1 Underlavment:
6.1.1 Underlayment shall be acceptable to Owens Corning and shall hold current Florida Statewide Product
Approval,or be Locally Approved per Rule 61620-3,per FBC 1507.2.3,1507.2.4 or R905.2.3.
6.2 Asphalt Shingles:
6.2.1 Installation of asphalt shingles shall comply with the Owens Corning current published instructions, using
minimum four (4) nails per shingle in accordance with FBC 1507.2.7 or R905.2.6, with the following
exceptions:
➢ Berkshire shingles require minimum five(5)nails per shingle.
➢ WeatherGuardo HP shingles require minimum six(6)nails per shingle.
➢ Devonshire"'shingles require minimum six(6)nails per shingle.
➢ Starter Strip Shingle and Starter Strip Plus require minimum five(5)nails per strip.
Refer to Owens Corning published information on wind resistance and installation limitations.
6.2.2 Fasteners shall be in accordance with the manufacturer's published requirements, but not less than FBC
1507.2.6 or R905.2.5. Staples are not permitted.
6.2.4 Where the roof slope exceeds 21 units vertical in 12 units horizontal, special methods of fastening are
required. See figures below for details.

6.3 Hip&Ridge Shinales:
6.3.1 Installation of Berkshire Hip and Ridge Shingles, High Ridge, WeatherGuard® HP Hip and Ridge Shingles
and ProEdge Hip & Ridge Shingles shall comply with the Owens Corning current published instructions,
using four (4) nails per shingle. Installation of DuraRidge'A Hip & Ridge Shingles shall comply with the
Owens Corning current published instructions, using two (2) nails per shingle. Refer to Owens Corning
published information on wind resistance and installation limitations, including the use of hand-sealing for
wind warranties.
6.3.2 Fasteners shall be in accordance with the manufacturer's published requirements, but not less than FBC
1507.2.6 or R905.2.5. Staples are not permitted.

7. LABELING:
7.1 Labeling shall be in accordance with the requirements the Accredited Quality Assurance Agency noted

7.2 Asphalt shingle wrappers shall indicate compliance with one of the required classifications detailed in FBC
Table 1507.2.7.1/R905.2.6.1.
8. BUILDING PERMIT REQUIREMENTS:
As required by the Building Official or Authority Having Jurisdiction in order to properly evaluate the installation of
this product.

THIS CONTRACT AGREEMENT, submitted between ROCK SOLID ROOFING,INC.,hereinafter
called the Roofing Contractor and BENEDUCE REALTY, INC. hereinafter called the Owner:

The Roofing Contractor shall supply all of the materials and perform all of the work in a professional manner

ARTICLE #1
The Work of This Contract
1.1 SCOPE OF WORK:
1.1.1 Remove the existing Shingle and Flat roof systems and replace with an Owens
Corning HD 130 mph. roof system and GAF 60 mil TPO roof system.
1.2 PROCEDURES:
1.2.1 Remove the existing layers of Shingles and Flat roofs to the plywood deck.
1.2.2 Inspect decking/non-visible fascia and L-flashing to receive new roof materials.
Damaged or deteriorated decking/non-visible fascia, truss work and L-flashing will
be brought to the attention of the Owner and billed on a material billing basis.
1.2.3 Remove the remaining portions of the existing eave drip and roof related metal
flashings and discard.
1.2.4 Dry-in sheathing with a Premium peel and stick (Secondary Water Barrier)
underlayment to the properly nailed plywood deck (6" O.C. using 8d ring shank nails)
per building code.
1.2.5 On the Flat roof,install a GAF 60 mil TPO membrane to the sloped insulated deck
using screws and plates per local wind uplift criteria.
1.2.6 Install new Lead boots, Goose neck vents, and Omni Low Profile ridge vent at
existing locations. Install new edge metal to the perimeter of the roof system.
1.2.7 Install Owens Corning HD dimensional shingle to prepared surface using 1 '/a"
stainless steel coil nails 6 pel�le. Hip and Ridge cap provided by the
manufacture. (Color to be Grey )
1.2.8 Clean up and haul away all roof related debris associated with the Scope of Work and
dispose of it in a legal manner.

ARTICLE #2
Insurance and Fees
2.1 A Building Permit will be provided by the Contractor for the execution of the Scope of Work based
on the Contract Amount.
2.2 A Statement of Insurance for both Liability and Workmen's Compensation will be supplied from the
Contractor's agent and reflect the necessary coverage as determined by governing local and state
agencies.
ARTICLE #3
Warranties
3.1 CONTRACTOR'S WARRANTY:
3.1.1 For a period of 5 years, the Contractor will warrant against water intrusion through the
roofing system. The warranty does not include damage as a result of abuse or negligence,
structural faults, severe weather including lightning,windstorms,hurricanes,acts of God or
situations not related to faulty workmanship.
3.2 MANUFACTURER'S WARRANTY:
3.2.1 Shingle Manufacturer will provide a limited lifetime warranty.
ARTICLE #4
Contract Sum
4.1 Owner shall pay the Contractor for performance of the Scope of Work, the sum of:
Ten Thousand Seven Hundred Sixty Dollars ($10,760.00)
Alternate #1: Install new Solar Roof Vents at $550.00 ADD Per Vent
4.2 The Contract sun is effective for 30 days and is subject to change without notice thereafter.
