below is not valid. For information to apply for anew food permit,contact the Florida Department of Agriculture and Consumer Services
at 1800-HELP FLA(1-800-435-7352)or visit our website at www.FreshFromFlorida.com.
The renewal fee for the Food Permit shall be due annually on January 1. It is the responsibility of the business owner to provide any
changes to the billing address. If the renewal fee.is not paid by the due date,a late fee of$100 must be paid in addition to the food
permit fee before the Department will issue the food permit as specified in Chapter 500.12(4)(b)1, Florida Statutes.
