AGREEMEN . For a period of e year effective (r' I ,,, :\Ql'Î through {~-lOcð¡~ G&C PEST CONTROL INC.
will provide neces ry treat t for the control of subterranean termites (Reticulitermes sp., Heterotermes sp.) for
the described prope e sum of $ \~ç" DO payable when services are rendered. Any further treatment
found necessary as a result of new live infestation of subterranean termites during the term of this plan , will be
performed at no additional charge.
G&C PEST CONTROL INC. will extend this limited guarantee annually to the purchaser upon payment of an annual
renewal fee of $ I:)o,O/J. payable on or before the end of the previous protection period. This Limited Guarantee
may be extended upon payment of annual renewal fee each year for an additional four years. Any further
inspections found necessary during the term of this plan will be performed at no additional charge. After the third
year and each year thereafter, G&C PEST CONTROL INC. reserves the right to adjust the annual renewal fee.
G&C PEST CONTROL INC. must be notified prior to any structural additions or alterations to the treated property.
Failure to notify G&C PEST CONTROL INC. will void this Limited Guarantee Agreement. Arrangements must be n
with G&C PEST CONTROL INC. for additional services at the purchaser's expense. G&C PEST CONTROL INC.
reserves the right to adjust the annual renewal fee in the event of structural additions or alterations.
The Purchaser understands that G&C PEST CONTROL INC.ls liability under this Limited Guarantee is limite
retreatment only, and does not guarantee against present or future damage to property or contents, nor provides for
repairs or compensation thereof.

GENERAL PROVISIONS
PURCHASERS RESPONSIBILITY: It is the Purchaser's responsibility to maintain
treated area during the life of this agreement from any conditions contributing to
infestation by subterranean termites; such as below grade stucco on exterior, excessive
moisture, wood in direct contact with ground, or faulty plumbing.
PROPERTY DAMAGE: The Purchaser acknowledges that G&C PEST CONTROL
INC. shall not be responsible for any property damage unless caused willfully or by gross
negligence, and that damages to real and personal property may occur in the course of
treatment even with the exercise of due care.
OWNERSHIP AND A GEN CY: By entering this agreement, the Purchaser represents
that he or she is the owner of the property upon which he the structure is located. If this
agreement is signed by an agent of the Purchaser, the agent represents that he or she has
the authority from the Purchaser to enter this agreement.
LIMITED GUARANTEE: This agreement does not control or protect against drywood
termites (Kalotermes sp., Insistermes sp., Cryptotermes sp.), Formosan termites
(coptotermes form 0 s anus ) or other wood destroying organisms such as carpenter ants,
powder-post beetles, wood decay fungi, etc. This agreement does not cover damage
resulting from subterranean termites to fences or wood decks.
LIMITED TRANSFERS: For a period of one year, this agreement may be transferred to
any subsequent owner of the described property. After this twelve month period, the
Purchaser may not transfer this agreement.
DEFAULT, NON-PAYMENT: G&C PEST CONTROL INC. has the right to terminate
this agreement in case of non-payment by the Purchaser. Purchaser will pay all attorney
fees and the cost of collection. For the period of delinquency, interest at the highest legal
rate will be assessed. A $25.00 fee will be charged by G&C PEST CONTROL INC. for
any check that is returned or not paid.
ARBITRATION:G&C PEST CONTROL INC. and the Purchaser agree that all matters
in dispute between them arising out of or relating to this Agreement shall be settled
exclusively by arbitration pursuant to the Uniform Arbitration Act of the American
Arbitration Association. The decision of the arbitrator shall be a final and binding
resolution of the disagreement which may be entered as a judgment by any court having
jurisdiction. Neither party shall sue the other where the basis of the suit is this Agreement
other than for enforcement of the Arbitrator's decision. In no event shall either party be
liable to the other for the indirect, special, or consequential damages or loss of anticipated
profits.
