Ladies and Gentlemen:
A. M. Engineering and Testing, Inc. is pleased to submit this report of our pile installation
monitoring for the referenced project. Representatives of A. M. Engineering was present on site
on August 1 and 2, 2005, to observe the installation of the cast -in -place auger piles.
Logs were kept of the pertinent information and are enclosed. The pile identification numbers
noted on the logs correspond to the numbers indicated on the enclosed pile layout plan The
number of "Required Strokes" was determined by measuring the pump strokes needed to fill a
container of known volume. The strokes necessary to fill the theoretical volume plus 10 percent
were then calculated to determine the required strokes for the individual piles. The pile lengths
noted on the logs are based on the depths below the existing grade at the time that the piles were
installed.
All of the monitored piles took grout equal to or in excess of their theoretical volume plus 10
percent.
Placement of the reinforcing steel for the piles was also monitored. The steel reinforcing was
constructed and placed in substantial compliance with the plans.
Concrete grout cubes were made for each 50 cubic yards of grout placed. A total of three sets of
cubes were molded. The results will be submitted under separate cover.

The following is in regards to the above project, all the stem walls, beams and garage floor were
inspected and were ready to pour. The fill in the garage between the stem walls and under the
slab was compacted and does not need a compactors test.

The reason we have not ordered the inspection is because of delays imposed on us by
others. Therefore, there is no need for the permit to expire.
