Rer your requesdl, AWARD Engineering, Inc. has designed a Rartial-Foundation Stabilization Program consisting of a
pressure pier underpinning system for the above referenced location. The repair work was performed by Helicon
Foundation Repair Systems, Inc. (CBC #125531CI). Following ccmpletlion of the work, AWARD Engineering inspected
the provided insdlallaticn logs and phctlosi. This letter summarizes the underpinning pier installation by Helicon.
The underpinning system was installed on June 11 and June 12, 2020. A total of iourteen (14) pressure piers were
installed at selected locations along 11he exterior walls of the structure including the southeast corner section, the
norihwesdl corner section, and in the pool deck area. Per the request of the homeowner, Pressure Pier SP12 wasi
relccailec from its proposed locallion. See attachec As-Installed Rlan for approximate pier locations. The pressure
piers were installec to ceptHs ranging from thirty-twc (212) to foray-fcur (44) feet below grcunc surface. Individual
installation records showing depth and applied pressure for each pier are also attached.
Basec on the prcviced information, it is the opinion of AWARD than Helicon Roundation Repair Systems, Inc. has
completed the installation of this Rartial-Fcuncaticn Stabilization System in accordance with generally accepted
industry standards and practices to the best of cur belief and knowledge and 11hat the foundation of the structure at the
repaired locations is stabilized at this time. The scope of this report does not cover any problems that may occur in
areas beyond the limits of this repair work or as a result of pre-exisdlinc, conditions.
The prcposec plan by AWARD Bncineering, Inc. was Iimillec 110 the iouncation stabilization of the structure at the
repaired locations, based on our client's request for a pressure pier uncerpinning plan. This remediaticn is nail
intendec to mitigate any deep soil conditions or to provide a remediaticn for any sinkhole sitatus that might exist at the
site.
