Following is the credit card authorization form and all information to renew 2 expired permits
I thought this information was faxed to you previously but obviously was not received. I have been
pretty ill and failed to follow up on these renewal requests
We have received a notice of violation For the property on 503 W midway Rd.
Please advise what steps will need to be taken to close these permits as both permits were total
demolitions for the Florida Department of Transportation Right away.
I believe that both sites have roads paved over the area where the demolition work was done.
