GFA International, Inc. (GFA) has been retained by Cenury Complete Homes to perform Private Provider Inspection
Services for 5711 Eastwood Dr. Lot 11 in St. Lucie County. The inspection services will be performed to verify that
the proposed structures are constructed in accordance with the approved plans, specifications, and any approved
modifications.
The qualifying Private Provider will be Thomas Montano (PE84146) who is competent to provide the inspection
services for these types of structures. The building inspectors licensed under FS468 in the various disciplines will
be Josean Duprey Rodriguez (BN7330), Don Determan (BN4688), John Carl Peterson (BN5555), William
McColman (BN7752), Richard Jefferson (BN1164), Richard Schofield (BN2710), Don Green (BN7301), John
Postlewaite (BN2543). The branch office that will be responsible for handling this project is located at 607 NW
Commodity Cove, Port St. Lucie, Florida 34986, the phone number is (772) 924-3575 and the fax number is (772)

GFA will be performing the required inspections required by St. Lucie County and the Florida Building Code. All
relevant inspection reports will be emailed to your office within (2) business days as per FS 553.791 (10).
