PLEASE SELECT ONE OF THE FOLLOWING:
CHANGE OF CONTRACTOR — Change of Contractor is to be signed and notarize le property owner,
and the new contractor of record for the current permit. A new permit application must also be completed with new
contractor information and signature. A new Notice of Commencement must be filed in the new contractor's name
for job values greater than $2,500 ($7,500 if A/C Change -out). A recorded copy must be submitted prior to
commencing any work. There is a $50.00 fee for the Change of Contractor.
CHANGE OF SUBCONTRACTOR — Subcontractor changes are to be completed by the general contractor.
The new subcontractor must fill out a Subcontractor Agreement Form. There is a $50.00 fee for the Change of Sub -
Contractor.
CANCELLATION OF PERMIT — The cancellation of a permit is acceptable only if no work has been done.
Cancellation of permit is to be signed and notarized by both the owner and qualifier of record. There is no fee for
cancellation of the permit.

We are simply exercising our protection rights via the lien, given you have not
conformed to the contract obligations. Final payment became due and owing at the time
of final install; more specifically when the last window was installed on the property. You
previously expressed some financial and personal hardships shortly after the installation
so we were lenient in the payment process.
I'm furtherance of our previous statement, please ensure the original permit application
is mailed to our corporate offices for processing. This will ensure the additional permit is
procured and we can thusly close out the project. Once this tasks has been completed
processing for approval should be approximately 2 weeks, thereafter we can host the
final inspections and close out any other services for the project. However, it's
withstanding that a financial commitment is required to take more action given the size
of the balance due and owing.
Please advise on payment remit as soon as possible and the status of the application.

Good Morning,
Your interpretation of the events and mine are obviously different and therefore are my
basis for my position. Seeing as you are the "large" company with the power to inflict
damage upon myself and family via liens against my home i obviously dont have much
power in the situation do i, because you are going to act in whatever means necessary to
get "paid".
What is your proposal? When will the repairs be completed on my property so we can
finalize the project? Please provide a DATE SET IN STONE, and lets work out what "financial
obligations" you would like to see on my end.

The series of events which have unfolded since the completed installation in July are
concerning and warrants our demand for payment prior to moving forward with any
further action which shall present additional costs to our company. During and shortly
thereafter the completed install, our team was unable to contact you for some time
regarding the payments due on your account. Eventually, we were able to arrange only
partial payment through a limited approval from a renowned home improvement
finance agency for your project. Since then we have had intermittent periods of
communication regarding the corrections need for the permitting of the site and the
remaining financial obligation on your account for the improvement s made by
Statewide.
We need some sort of financial commitment as a sign of good faith you will remain
cooperative moving forward towards closure on this project and the remittance of final
payment.

Mr. Moura, I received your message Friday morning and attempted to call you back in the afternoon
once I returned to the office. Just to give you a summary of the remaining tasks for permit closure
please see below:
• The building inspector requested a structural detail on the renovations for the one window
opening which was altered in size.
• Statewide had the associated plans developed by our engineer disclosing the method of work
• After an inquiry with the inspector directly, he requested the plans be processed as a revision
prior to reviewing upon the next inspection
o These plans are currently in process with ,Fort Pierce. Once approved we will. reschedule
the inspection with the building official
• Once the inspector has performed the review of the structural plans we can then tinishes off the
drywall. around the opening and remove the product labels from the windows
As a separate matter, the residual balance on your account still remains outstanding and owing. I
believe you wanted to discuss payment options to reconcile the remaining balance.
Please email me directly with your intent on the payment remit moving forward so we may review.
