Dear Mr. Thomas:
GFA International, Inc. (GFA) has been retained by T-Mobile to perform plan review and inspection services for
their tower located at 16800 Okeechobee Road in St. Lucie County. The inspection and plan review services
will be performed to verify that the proposed structures are constructed in accordance with the approved plans,
specifications and any approved modifications.
The qualifying Private Provider will be Thomas Montano (PE 84146) who is competent to provide the inspection
services for these types of structures. The plan reviewers licensed under FS468 in the various disciplines will be
Josean Duprey (PX4208) and Walter Pride (PX3512). The building inspectors licensed under FS468 Josean
Duprey (BN7330), William McColman (BN7752), John Postlethwaite (BN2543), Don Green (BN7301) and/or James
Faidley (BN7717). The branch office that will be responsible for handling this project is located at 607 NW

GFA will be performing the required inspections required by St. Lucie County and the Florida Building Code.
All relevant inspection reports will be emailed to your office within (2) business days as per FS 553.791 (10).
