As requested, enclosed are three (3) Design Certification for Wind Load
Compliance (signed and sealed by the Engineer of Record), three (3) copies of Lintel
Specifications, and three (3) copies of Dade County Product Approvals for DSL Panel
Systems. As required, these items complete the initial building permit No.21020528
submittal.

Pursuant to our conversation on 02/21/01, please .accept this Building Permit
Application submittal complete with a check in the amount of $175.00 made payable to
St. Lucie County, a complete application for building Permit, three (3) sets of building
plans signed and sealed by the Engineer of Record and a copy of the NOC.
