Mr. Cicio, as of our last meeting where you assisted me in assembling a revision application I have been having
difficulties processing the revision through the Health Dept. due to Mr. Duncan's illness. I met with him this morning and
he is having difficulties locating the original permit and he is setting up a meeting with Mr. Turner, the reviewer of the
original permit to resolve this issue. This has delayed me from meeting the 6 months inspection requirement and I ask
for consideration an extension of-30=days to-comply=with-t-HFnecessary=inspections. I would greatly appreciate your
assistance in the mater. Sincerely, Gary Kernan Cell Phone # 772-486-8818
