To Whom It May Concern:
This firm was retained to certify the installation of a"5V"crimp metal roofing that was completed without having
required intermediate inspections at the above referenced project.This report and all statements are based on orL-site
inspection of randomly chosen section,photo and/or video documentation taken during installation,paid invoices,
and delivery receipts provided by the owner,Mr.Steven M.Trempe. Our findings are as follows:

Documentation:
Mr.Trempe proved to our satisfaction that they did indeed utilize"5V"Crimp metal roofing manufactured by
Extreme Metal Fabricators,LLC(installation governed by product approval FL 17022.1),and Ultra HT Wind&
Water Seal underlayment manufactured by MFM Building Products Corp.(installation governed by product
approval NOA 18-1015.01)delivered to the site to cover the project's roof.
We were provided photographs sufficient to verify the following Construction:
I. Wood Plank Decking
!wry We verified existing lx6 wood plank decking was free of damage and was re-
nailed throughout with a minimum of(3)10d ringshank nails @ each truss
bearing.

II. Underlavment
We found that Ultra HT Wind&Water Seal underlayment was installed that
utilizes a self-adhered fastening system. There was no unexplained stretching,
' bubbling,wrinkling,and/or tears with all proper edge/end lapping maintained.
As such we are certifying that the underlayment was properly installed.
III. "5V"Metal Panels
We verified the use of 5V"Crimp metal roofing manufactured by Extreme Metal
Fabricators,LLC and fastened with#1Ox1-1/2"wood screws w/sealing washers
z with row spacing of 16"o.c.and @ 12"o.c along panel width.
Final Findings:
All inspected areas of work were installed in accordance with the 2017 Florida Building Code 61 Edition,all
product approvals,and in compliance with acceptable construction practices. All fastening is compliant with the
design pressure determined by this firm. Therefore,we hereby certify the installation of the"5V"metal roof panels
and all required components meet or exceed all requirements of the 2017 Florida Building Code 61 Edition.
