US Highway 1, Port St. Lucie, FL 34952 Observed By: Noel Ramirez, P.E. Kimley -Horn Date of Observation: 12/07/2020 305-535-7705 Time of Observation: 1:00 p m noel.ramirez@kimley -horn.com Date of Report: 12/17/2020 Temperature / Weather: 85o, Sunny Present on Site: Noel Ramirez Grant DeLa Vasselais Jean-Simon Berube Kimley -Horn & Associates (KH) Structural Structural Activities Observed: 1. Structural and Waterproofing Repairs Kimley -Horn conducted a site visit on December 7, 2020 to observe structural and waterproofing repairs on the Best Western Hotel – Balcony Restoration project. Upon arriving on site, progress work was observed on the NE and NW building blocks. The following observations were noted: a. Shoring was installed in all areas where repair work was taking place following the approved shoring shop drawings and recommendations from Kimley -Horn on the previous visit . (Reference Photos 1-2) b. Progress was observed on spall areas where delaminated concrete had been removed and concrete edges sawcut at 90 degrees. Concrete removal and surface preparation were observed to be in accordance with the restoration plans and technical specification in most locations keeping concrete removal configurations with rectangular shapes and square corners . Concrete removal was ex tended along the reinforcing steel until there was not further delamination, cracking or significant corrosion. Where half or more of the perimeter of reinforcing bar was exposed, bond between the reinforcing bar and surrounding concrete was broken, or the reinforcing bar was corroded, concrete was removed from the entire perimeter of the bar to provide at least 0.75 in. clearance between exposed reinforcing steel and surrounding concrete. Locations with lack of concrete removal around reinforcement were id entified by Kimley -Horn and marked by the contractor in the field. Kimley-Horn will check these areas on the next site visit to verify compliance with the drawings . (Reference Photos 3- 5) c. Kimley -Horn observed reinforcement with significant corrosion and loss of cross-sectional area in several locations. Areas were identified by Kimley -Horn and marked by the contractor in the field . Contractor shall notify Kimley -Horn where significant amount of material has been lost due to corrosion. Where section loss of reinforcing bars is more than 20% of the cross - sectional area, splice replacement bars to existing bars as directed by Kimley -Horn. Remove additional concrete as necessary to provide at least a 0.75 in clearance beyond existing and replacement or supplemental bars. Kimley-Horn will check these areas on the next site visit to verify bars were properly spliced. (Reference Photos 6-7) d. Reinforcement shall be cleaned and reviewed by Kimley -Horn before applying bonding agents and patching material. Kimley-Horn requested the contractor to p rovide support chairs, and holding bars properly spaced and with sufficient strength to carry loads of reinforcement and deposited concrete without collapsing or allowing bars to sag. (Reference Photo 8) e. All corroded or damaged electrical conduits shall be repaired/replaced before ap plying the patching material. f. Structural identified an overhead area with concrete honeycombing at the west balcony slab of the NW block. Also, it was noticed that partial-depth repairs are being performed on the CONSTRUCTION OBSERVATION REPORT 2 Best Western Hotel – Balcony Restoration Page 2 surface above. By the time all layers of concrete honeycombing are removed until suitable concrete is exposed, the thin concrete layer above would potentially break or crack. Therefore, Kimley -Horn recommended a full-depth type of repair along the honeycombing area.
