This firm was retain to certify the installation of a flat roof that was completed during the re-roof of the
above referenced project was done in accordance to our design submitted to your department for review.
The following is what our associate found:
Plywood Decking
All compromised plywood decking removed and replaced with 19/32"
plywood. Entire roof plywood decking re-nailed with 8d ringshank nails 6"
O.C.thtough-out.
Base sheet:
Documented proof that low slope CertainTeen Glasbase sheet with min 4"
lap fastened with 11 ga.Ringshank nails with 1-5/8"'dia tin caps spaced 4"
O.C.in lap and 3 staggered rows 6"O.C. exceeding the requirements of the
Florida product approval(FL10497-R5)and our design as submitted to the
city.

Cap Sheet:

Visually inspected Torch applied cap sheet. Not bubbles under cap,no shifting
in evidence,or compromised sheets.
With these observations we are able to certify that the low slope roof installation of this re-roofing repair
meets and/or exceeds our design as submitted to your department,the 2014 FBC 5`"Edition,Florida
Product Approval FL-10497-R5,good construction practices, and all local interpretations of the FBC and
local ordinances.
Addendum:
On March 23,2017 your department issued review comments on our last letter. Our responses are as
follows:.
1. Corrected spelling of street name on letter above
2. Product approval properly marked and identified has been added to this letter.
a. Noted Spacing of base sheet is good for a ASD pressure-90.0 far in excess of the
required pressures of-Zone 1: -27.6,Zone 2:46.4,Zone 3:769.8 as required by the 2014
FBC 5t'Edition for flat roofs less than 7 degrees in a 160 exposure B wind zone
3. As per our conversation a pull out test is not required since the entire roof was renailed as
observed and listed in our original letter
With these corrections and addendums we believe that the work and our description of it now meets all
requirements of the 2014 FBC 5t'Edition
