Subject: Site Development Permit CHRIS DZADOVSKY Wallaby's Fabrication, LLC, Phase II VICE-CHAIRMAN DISTRICT 1 Ref: SPMj-2201915479 SEAN MITCHELL The Construction Plans signed and sealed December 12, 2019, are accepted for DISTRICT 2 construction of the site improvements as proposed. Construction of site improvements is conditioned on the following: LINDA BARTZ 1. This permit is for phase II new improvements, understanding some minor DISTRICT 3 modifications are to be implemented within the Phase I, existing area. 2. Prior to commencement of any construction of site improvements, the FOR shall coordinate a pre -construction meeting including the St. Lucie FRANNIE HUTCHINSON County Public Works representatives. Please coordinate through David DISTRICT 4 Hays at 772-462-1491, haysd@stlucieco.org 3. A set of plans stamped as accepted by the County, together with all other HOWARD N. TIPTON applicable permits shall be on site at all times during construction. COUNTY ADMINISTRATOR 4. Prior to commencement of work, all other agency permits must be obtained and followed. The more stringent condition of common or conflicting conditions shall be met. Discrepancies within the plans shall be DAN MCINTYRE appropriately addressed and does not relieve the contractor of his COUNTY ATTORNEY obligation to construct according to local, state and federal rules and regulations. Inspections shall be performed at a minimum for the FORT PIERCE, FL 34982 following stages: A. Delivery of pipe and structures prior to installation PHONE B. SW structure installations and backfill (772) 462-1 707 C. Subgrade compaction testing and stringline D. Base compaction and stringline TDD E. Asphalt/paving (772) 462-1 428 F. Final: after FOR Certification and receipt of as -built survey 6. Any proposed changes to the permitted stamped plans must be approved FAX by the County prior to initiating any change in construction. This includes (772) 462-2362 material changes such as pavement type, type of drainage pipe, etc. 7. Any structure shown or not shown on the plans (such as walls, gates, signs, columns, etc.) requiring a building permit, such permit shall be obtained E-MAIL CHAM BERSG@STLUCIECO.ORG prior to initiating construction on such structures. The permittee is responsible for administering the construction contract in accordance with this permit. If the permittee fails to comply with any part of this permit or any other permit issued for this project by any regulatory authority, the County may at its sole discretion, cause to cease construction activities on the site until said permit condition is complied with. A Cease and Desist Order may be issued by the County Administrator or his designee, and served in an appropriate manner to the permittee or the permittee's agent. Under no circumstances shall work proceed when a Cease and Desist Order has been issued until said Order has been withdrawn in writing by the County. If a Cease and Desist Order is issued, the County, at its sole discretion, may take other action against the permittee and the permittee's agent(s) for the project. 9. Activities approved by this permit shall be conducted in a manner which does not cause pollution or allow pollution to exit the site. The permittee is solely responsible for preventing pollution caused by dewatering water and/or stormwater runoff from the construction site or work area. In addition to taking its own actions and remedies against the permittee for a permit violation, the County may report each violation to the South Florida Water Management District, Florida Department of Environmental Protection, Water Control District (or other Florida Statute Chapter 298 Drainage District, as appropriate), County Code Enforcement, and other pertinent regulatory or enforcement agencies. No construction work shall begin until the pollution control and treatment system have been constructed in accordance with approved permits and plans. 10. A copy of the Stormwater Pollution Prevention Plan (SWPPP) must be available on site during construction. Any necessary change to the SWPPP shall be agreed upon with the County prior to implementing such change. 11. An `Illicit Discharge Sign' shall be obtained, installed and remain posted continuously during construction activities. The sign may be obtained from the County Public Works Department at the pre - construction meeting or by separate arrangement's. 12. An As -Built survey is required along the EOR's certification upon request for a final inspection of the site and access improvements. A published benchmark utilized for site elevations shall be referenced for use by other parties, including but not limited to the County.
