WORK SCOPE: Re -Roof Residence about 20 Square with Mill Finish 5V Metal Roof System.
Remove one layer of shingle roofing down to plywood. Discard all debris legally off site.
2. Examine plywood nailing pattern, if not nailed to code bring up to code.
3. *Add Restrapping of the rafters to code, materials and labor cost $900.00
4. Furnish and install Peel N Stick High Temp underlayment to code. $800.00
5. Furnish install multipurpose vents (mpv) and pipe boots on all plumbing vents.
6. Furnish and install new metal eave drip edge, mill finish valley and flash others, to code.
7. Furnish and install 26G Mill Finish 5V Metal Roof System, to code. (exposed fastener)

Work is outlined in the scope above requests outside of the scope will be considered an Add. The roof has been determined
to be about the number of squares noted above, this number is fixed and will not go up, the scope is for removing one layer of
shingles to the plywood deck. Then re -nailing deck to code as needed.

■ Additional work (if needed), beyond the scope of our contract will be charged at material cost plus 25%
and labor rate of $65.00 per man-hour.
The work will be accomplished and guaranteed in accordance with industry Standard General Conditions
and Limited Warranty, which are made a part of this Proposal and Agreement and incorporated herein by reference.
All labor and material necessary to perform the above work will be furnished for the sum of:
Eleven Thousand Six Hundred Dollars ( $11,600.00)
Deposit of 35% of the contract amount due upon acceptance of the owner
Fifty (50) Percent due upon roof removal and dry -in.
Balance due upon completion

Prior to initiation of any action, the parties agree to participate, in good faith, in a minimum half -day mediation with a mediator
either chosen by agreement between the parties or as assigned by the American Arbitration Association. In the event the dispute
is not resolved by mediation, any claim or controversy arising out of or relating to this Agreement or breach thereof, or to any
action by an employee or agent of contractor, shall be settled by arbitration in accordance with the Federal Arbitration Act and
Construction Industry Arbitration Rules of the American Arbitration Association. However, the arbitrator shall be required to
issue written findings of fact and conclusions of law. Moreover, the parties shall split the cost of keeping a written record of all
proceedings. The judgment of the Arbitrator may be entered in any court having jurisdiction thereof and shall be final and
binding on both parties, except for errors of law, which shall be appealable. The parties agree that all expenses of arbitration or
mediation will be shared equally or as awarded by the arbitrator. The successful party shall also be entitled to attorney fees and
all costs of litigation.
