Dear Sir or Madam:
On Octuber 16, 2015 Mr. Ariesp Mejia of CALTROP Corporation performed a review of field
documentation of the T-Mobile telecommunication facility located at the above -referenced site. Mr.
Mejia regularly performs this kind of work, under the supervision of Mr. Michael A. Phillips, P.E. The
purpose of these observations was to inspect the following in accordance with the 2010 Florida Building
Code, Section 2218.2:
Connection of three proposed antennas.
Connection of three proposed Remote Radio Units (RRUs).
Mr. Mejia reviewed the documentation consisting of a construction photographs depicting the connection
of the above mentioned installation(s) and compared them with the construction drawings prepared by
Morrison Hershfield Corporation dated October 13, 2014. Based on the observations by Mr. Mejia, to the
best of our knowledge, belief, and professional judgment, the connections of the installed equipment were
performed in substantial compliance with these drawings.
