UTILITY will provide water and/or sewer service upon payment of the applicable connection charges and compliance with the Terms and Conditions listed on the back of this Service Application.The Utility policies provide for a water
and/or wastewater plant capacity allocation of two hundred and fifty(250)gallons per day equivalent residential connection(ERC).The Utility will not be obligated to provide capacity of service in excess of the allocation and may

CONDITIONS -
Applicant understands the following instructions and responsibility before the grinder system
installation. All scheduled installations are subject to change due to weather and other Utility
emergencies. For additional questions and/or clarification, please email us at
utilitysewer@cityofpsl.com.
1'. Instructed as to location of: Sewer Main: Front between lots

of the garage, no more than five feet from the corner of the home

2. Lot must be clear of all construction debris, lot rough graded and house stucco completed. Final
grade and sod cannot be done until tank set is completed.
3. If sewer stub-out is on the same side as the water meter, a 3" PVC sleeve 20' back from garage
door is required 18" under driveway to install the Utility's 11/2" service line.
4. The power must be supplied to the home's main panel. The power supply for the control panel,
i from the house service, must be a double-pole 20-amp breaker with four (4) #12 conductors,
i which will Include ground wire. The wire shall be 18" above finished floor and located on the side
of the home within five feet of the front corner (same corner as sewer lateral). Height of panel
shall be a minimum of 31/2 feet or 42 inches and a maximum of 41/2 feet or 54 inches.
5. Accepts responsibility to email City of Port St. Lucie Utility Systems Department at
utilitysewer@cityofpsi.com when the pre-inspection items are ready. If inspection fails, there will
be a $25.00 fee for re-inspect. Contractor is required to call back for 2"d Pre-Inspection when the
deficiency has been resolved.
6. Acknowledges the responsibility to correct all deficiencies at applicant's expense and eliminate
any conflicts in the way of Utility tank lines.
7. Acknowledges responsibility for inspection fee of $25.00 for each follow-up pre-inspection visit
required to satisfy non-compliance items.
