Dear Mr. Cicio:
As FOR I have completed the inspection related to the areas under the requirements of
the FBC 2014, 5th Edition, Mechanical in the reference address with the following
comments:
1. The permit card was exposed in working place
2. Energy Calculation according with the equipment installed
3. Notice of commencement in field
4. The duct sealing inspection was done by the Sub-Contractor DIAZ AND
RUSSEL, who certified in a written document attached.
5. The condensate drain and pipes and refrigerant lines, working properly
6. No vibration of the equipment and well insulated
7. Temperature remains daily in 750 Fahrenheit Degrees
8. Mounting compressor fastened accordingly
9. Bathrooms fan per approved fan schedule
10. Smoke detectors interconnected , typical units Kidde hardwire
11. Equipment as per equipment schedule sizes and calculations
12. Thermostat working properly after Battery replacement

Dear Mr. Cicio:
As FOR I have completed the inspection related to the areas under the requirements of the FBC
2014, 5th Edition, Building in the reference address with the following comments:
1. The permit card was exposed in working place
2. Notice of commencement in field
3. Slab in concrete of+/- 3000 psi, is existing and looks well poured of 6" supporting
masonry, interior partitions and it was open to install the plumbing pipes
4. Concrete Columns and CBS cells fill with concrete @ max. 48" O.C. per the plans
approved
5. Termite treatment done by SHIP SHAPE PEST CONTROL and a certificate about it
attached to this inspection report, in the slab opening,for a new bathroom.
6. The Drywall was attached with drywall screws to 1"X2" wood_furring on top of CB
walls @24" O.C. basically as sill of new windows.
7. The CB walls in blocks 16"X8"X8"with reinforced ladder typical @ 8" O.C.
8. The Insulation on CB walls was done with Batt R-5 between furring strips on the
interior side of the exterior walls.
9. Windows and doors were not changed and I requested to the General contractor to
install metal storm panels in new windows. A new sheet will be submitted for
approval.
10. Windows and doors flashing as part of the aluminum windows installation shows on
plans
11. Windows and doors as per plans

1. The permit card was exposed in working place
2. Notice of commencement in field
3. Drainages working properly.
4. No piping leaks detected
5. Vent pipe on top of roof well installed (6" above roof per FBC-R 3103.1) and
as per plans
6. Fixtures well installed and working as expectations —Minimum clearances
7. Clean out outside adequate in size and separation of the property and
accessible
8. Shower with proper slope
9. Anti —scald shower diverters
10. Water heater drain pan line/size termination point
11. Accessible valves installed
12. Correct minimum pipes sizes of water lines
13. Proper grade of pipes
