To Whom It May Concern,
Based on photographs and signed as-built construction documents provided to CBVR
Telecom Design Group by the authorized field agent (J. Crompton Electric, Inc.) on
January 20, 2020, I hereby attest to the best of my knowledge, belief, and professional
judgment that the installation of wireless communications facility and equipment for
the aforementioned project was constructed in general conformance with the approved
civil construction documents by Waypoint Engineering dated August 29, 2019:
Scope of project:
• Removal of existing vegetation to clear the construction site and access driveway. • Installation of asphalt apron and compacted access driveway to the telecommunication compound. • Installation of guyed tower and guyed anchor foundation. • installation of the 250’ guyed tower with the guyed wires and lighting system. • Installation of the antenna mount bracket on the tower. • Installation of the antennas and RRUs (remote radio units) on the antenna mount. • Installation of the electrical h-frame with multimeter bank. • Installation of the underground electrical conduits and grounding system.

• Installation of the elevated steel equipment platform. • Installation of the equipment platform concrete foundation. • Installation of the equipment on the elevated platform. • Installation of a new propane generator on the new equipment platform. • Installation of the propane tank on a new concrete slab. • installation of the chain link fence compound with 12’ wide chain link fence access gate. • Installation of landscaping.
The applicable design compliance codes for this installation include:
• Florida Building Code 2017 (6th Edition)

described above. Furthermore, this statement shall not constitute approval of any
safety precautions or construction procedures implemented during the construction.
