This product as described has demonstrated compliance with Florida Building Code 2020,
1504.3.2 (non-HVHZ) as required by FL Rule 61G20-3, method 1D.

Design Process:
The load tables in this report provides one prescriptive option for the fastening requirement
for the applicable wind loads for roofs within the parameters described. For roofs outside of
the listed parameters, design wind loads shall be determined as required by FBC 1609, ASCE 7, or
other design code in force, using allowable stress. These load tables are based on ASCE 7-16.
Use of these tables assumes that the structure is:
Enclosed and conforms to wind-borne debris provisions and is a regular shaped building
• Is not-subject to across-wind loading, vortex shedding, or instability; nor does it have a
site location for which channeling or buffeting warrant consideration
i Engineering analysis may be completed by other licensed engineers for project specific
approval by local authorities having jurisdiction.
Certification of Independence:
David Eng, PE and Timberlake Cove, LLC do not have, nor will acquire a financial interest in any company
manufacturing or distributing products under this evaluation. The same entities do not have, nor will
acquire, a financial interest in any othe, entity involved in the approval process of the product.
Exclusions and Limitations:
Design of deck and roof structure (to inciude attachment of plywood or wood plank) shall be completed by
others. Fire classification and shear diaphragm design are outside the scope of this evaluation. Accelerated
weathering/salt spray is outside the scope of this evaluation.
This report is limited to compliance with structural wind load requirements of FBC 1504.3.2, as required by
Rule 61G20-3. Neither Timberlake Cove nor the manufacturer shall be responsible for any conclusions,
interpretations, or designs made by others based on this evaluation report. This report is limited solely to
documenting compliance with Rule 61G20-3, and makes no express or implied warranty regarding
fR• performance of this product. Installation shall be subject to the local building code and authority having
jurisdiction; this report shall not be construed to supersede local codes in force.

This NOA is being issued under the applicable rules and regulations govern .ng the use of construction materials. The
documentation submitted has been reviewed and accepted by Miami-Dade County RER-Product Control Section to be
used in Miami Dade County and other areas where allowed by the Authority Having Jurisdiction(AHJ).
This NOA shall not be valid after the expiration date stated below.The Miami-Dade County Product Control Section
(In Miami Dade County) and/or the AHJ(in areas other than Miami Dade County)reserve the right to have this
product or material tested for quality assurance purposes. If this product or material fails to perform in the accepted
manner,the manufacturer will incur the expense of such testing and the AHJ may immediately revoke,modify, or ,
suspend the use of such product or material within their jurisdiction. RER reserves the right to revoke this acceptance,
if it is determined by Miami-Dade County Product Control Section that this product or material fails to meet the
requirements of the applicable building code.
This product is approved as described herein,and has been designed to comply with the Florida Building Code
including the High Velocity Hurricane Zone of the Florida Building Code.

LABELING: Each unit shall bear a permanent label with the manufacturer's name or logo, city, state and following
statement: "Miami-Dade County Product Control Approved",unless otherwise noted herein.
RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been no change
in the applicable building code negatively affecting the performance of this product.
TERMINATION of this NOA will occur after the expiration date or if there has been a revision or change in the
materials,use, and/or manufacture of the product or process. Misuse of this NOA as an endorsement of any product,
for sales, advertising or any other purposes shall automatically terminate this NOA. Failure to comply with any section
of this NOA shall be cause for termination and removal of NOA.
ADVERTISEMENT: The NOA number preceded by the words-Miami-Dade County, Florida, and followed by the
expiration date may be displayed in advertising literature. If any portion of.the NOA is displayed, then it shall be done

INSPECTION: A copy of this entire NOA shall be provided to the user b. the manufacturer or its distributors and
shall be available for inspection at the job site at the request of the Building Official.

LIMITATIONS:
1. Fire Classification is not part of this acceptance.
2. This acceptance is for prepared roofing applications.Minimum deck requirements shall be in compliance with
applicable building code. Ultra HT Wind&Water Seal"High Tempu::ature underlayment shall be installed in
strict compliance with applicable Building Code. '
3. Ultra HT Wind&Water Seal""High Temperature underlayment may )e used in mechanically fastened metal
roofing, asphaltic shingles,wood shakes and shingles,quarry slate roof assemblies. Ultra HT Wind&Water
Seal'High Temperature underlayment shall not be used as roof tile underlayment.
4. Ultra HT Wind&Water Seal""High Temperature underlayment shall be applied to a smooth, clean, and dry
surface with deck free of irregularities.
S. Ultra HT Wind&Water Seal""High Temperature underlayment shall not be applied over an existing roof
membrane as a recover system.
6. Ultra HT Wind&Water Seal"High Temperature underlayment shall not be left exposed as a temporary roof for •:'

9. When applying the membrane in the valley, start at the low point and work to the high point,rolling the
membrane from the center outward in both directions. For ridge applications, center the membrane and roll from
the center outward in both directions.
10. Roll or broom the entire membrane surface so as to have 100%contact with the surface,giving special attention
to overlap areas. •'y
11. Flash vent:pipes,stacks, chimneys and penetrations in compliance with Roof Assembly current Product Control
Notice of Acceptance and applicable Building Code.
12. All protrusions or drains shall be initially taped with a 6"piece of underlayment. The flashing tape shall be
pressed in place and formed around the protrusions to ensure a tight fit.A second layer of Ultra HT Wind&
Water Seal"High Temperature underlayment shall be applied over th;:underlayment.
13. All products listed herein shall have a quality assurance audit in acco dance with the Florida Building Code and
Rule 61 G20-3 of the Florida Administrative Code.
14. All membranes or packaging shall bear the imprint or identifiable marking of the manufacturer's naive or logo,
city and state of manufacturing facility, and the following statement: "Miami-Dade County Product Control
