Permit No. 2012-0537 To Whom It May Concern: This letter shall serve as certification that to the best of our knowledge, the structural repairs have been constructed in substantial conformance with the design documents and specifications and as duly noted in our field reports. Threshold Inspections for the above referenced project were performed from January 19, 2021 through May 6, 2021. Copies of all field reports were distributed to the Client, Engineer of Record, Contractor and to the St. Lucie County Building Department. If you have any questions, please call our office. A CONCRETE REPAIRS 5155 N. HWY. A1A, FT. PIERCE, FL 34949 SLC BUILDING PERMIT NUMBER: 2012-0537 PROJECT NAME: Ocean Harbor N. Condos – Bldg. A Concrete Repairs DATE: January 19, 2021 CLIENT: Elliott Merrill Community Management LOCATION: Ft. Pierce, FL CONTRACTOR: Structure-Con PROJECT MGR: Mr. Khris McConnell INSPECTED BY: Todd Rhoten WEATHER: Sunny WORK INSPECTED: Units 314 & 714 – Concrete demo / pre pour inspection of the reinforced concrete balcony. OBSERVATIONS: • The loose and deteriorated concrete along the balcony edge has been removed which was the entire length of the balcony, approximately 12 feet. • The contractor continued to chip until clean reinforcing was found which was approximately 18” from the slab edge. • The bars appear to be in serviceable condition and will require power tool cleaning and coating with the specified corrosion inhibitor. • Post shores placed as indicated in plan and form boards are in place. • The repairs are in accordance with design documents and approved for concrete pour. ______________________________ _________________________________ Michael Lue, P.E. Units 314 & 714 – Post pour. OBSERVATIONS: Unit 214 • Hurricane shutter tracks have been removed from the top and bottom of the slab. • The screen enclosure was removed at Unit 114 below. • Balcony edge demo is complete and the corroded reinforcing has been exposed. • The bars appear to be in serviceable condition and will require power tool cleaning and coating with the specified corrosion inhibitor. • The area of edge repair was limited to the south corner only, approximately 3 square feet. Unit 314 • Waterproofing and decorative coating has been applied to the repaired areas. • Stucco at the repaired areas has been applied to match existing and painted with a primer. • Hurricane shutter tracks have been re-installed and the top and bottom of the slab by the shutter contractor. Unit 514 • The existing screen enclosure has been removed. • Hurricane shutters did not require removal. • The balcony edge demo is complete and the corroded reinforcing has been exposed. • The bars appear to be in serviceable condition and will require power tool cleaning and coating with the specified corrosion inhibitor. • Area of repair was limited to approximately 2 square feet at the edge. Unit 614 • Hurricane shutter tracks have been removed from top and bottom of the slab edge. • The balcony edge demo is complete and the corroded reinforcing has been exposed. • The bars appear to be in serviceable condition and will require power tool cleaning and coating with the specified corrosion inhibitor. • Area of edge repair was the entire length of balcony, approximately 12 feet. Pierce, FL CONTRACTOR: Structure-Con PROJECT MGR: Mr. Khris McConnell INSPECTED BY: Todd Rhoten WEATHER: Sunny WORK INSPECTED: Units 214, 314, 514, 614 & 714 – Final Inspection OBSERVATIONS: • The repaired areas have new waterproofing, top coat and paint to match surrounding areas. • Hurricane shutters have been reinstalled along the slab edge. • Stucco has been applied to the ceiling and slab edge at the repaired areas. • All repairs are in accordance with permitted design documents.
