' a. Authorized Representatives of an Engineer or an Engineer performing threshold inspections shall
conturm to Rule61G15-30.004 of the F.A.C. & FL SEtute Section 553.79(5Xa),(b) and (c), Sections 110.7 & 110.8 of
the Floilda Building Code (FBC)
b. Special inspectors perbrming building department required inspections shall conform to the 8th
Edition (2017) Florida Building Code, comply with Section 110.8, and be licensed under FS 468 PartXll. Permission
to perficrm these inspec'tions may only be performed under specific authorization of the Building ffidd

building official, the architect, or the engineer of record

l, (we) understand that a Special lnspection Log for each building must be displayed in a
convenient location on the site for reference by the St. Lucie County Building lnspector. All

mandatory inspections, as required by the Florida Building Code, must be performed by the
county. The st. Lucle county Builcling lnspector must be called for all mandatory lnspecfions.
lnspections performed by the Special lnspector hired by the Owner are in addition to the
mandatory inspections performed by the Building Department. Upon completion of the work
under each Building Permit, I (we) will submit to the St. Lucie County Building lnspector at the
time of final inspection the completed inspection log form and a signed and sealed statement
indicating that, to the best of my knowledge, belief and professionaljudgment, those portions of
the project approved for special inspections and permits outlined above and are in substantial
accordance with the approved plans. I (we) hereby certify that if shoring is needed to be
installed on an emergency basis, it will be in compliance with the provisions of the Florida
Building Code and in conformance with generally accepted engineering standards.

This letter is to inform you as threshold building special inspector, John Breitenbach PE 59770, Sl
#2072 will be performing Threshold building services for the proposed Concrete Restoration
Project as outlined in the Plans and Specifications of CSM Engineers, dated October 2020.
The specifications include the Scope of Work and a general lnspection Plan provided by the
Engineer of Record (CSM Engineers).
I will be sending you copies of the final inspection sumrnary reporUor final completion certification
letter when the project is completed. The following is the inspection plan (generalized & short):
1. lnspection of concrete surface after demolition or spall removal. (progress)
2. lnspection of shoring as required; re-shoring as required.(progress)
3. Final inspection of forming, bar repair, bar coating, bar cover, and final concrete pouring.
A monthly report progress inspection report will be issued for larger jobs that have two or more final
work area inspections per month with a copy to the Contractor. The Contractor is responsible for
reporting to the building department.
Note that the Special lnspector does not perform continuous detailed records of work performed.
Detailed records such as photographs, measurements, quantities, Contractor oversight and as-buflfs
are the responsibility of the Engineer of Record.

This letter is a proposal for perforrn the special inspection services for the above project as required
by the Florida Building Code. Services generally include the following:
1. lnitial Letter of lntent to the local building department announcing procurement of special
inspector services for the project.
2. Field inspections of demolition, shoring, reinforcing bar cleaning or replacement, bar coating,
and forming.
3. Coordinate with the Engineer of Record for field decisions concerning re-bar placement and
extent of demolition.
4. The frequency of inspections is determined by the size of the project and the speed and
progression of the Contractors work schedule and man-power.
5. Final pre-pour inspection the day of (or day before) concrete placement.
6. Final Letter of Certification that the Contractor has performed the work according to the
Florida Building Code and the Engineer's specifications and drawings.7. lnspection reports will be forwarded once every month to correspond with the Contractor or
Engineer's billing cycle. All reports are signed and sealed by the Special lnspector.
