We are pleased to submit our proposal for the following: Concrete restoration repairs in accordance with ENGINEER'S and/or
INTERNATIONAL CONCRETE REPAIR INSTITUTE specifications and procedures
Painting of repaired areas if required is not included in the estimated cost unless itemized by a unit price which will be quoted
if requested.
This proposal provides an estimate based on visual inspections. Actual quantities may increase or decrease and are based on
the following fixed unit prices and estimated quantities as follows:

WHENEVER POSSIBLE WORK WILL BE ACCOMPLISHED FROM OUTSIDE VIA SCAFFOLDING AND/OR
LADDERS. WHEN ACCESS MUST BE THROUGH THE APARTMENT, A CLEAN RUNNER WILL BE PLACED
FROM THE FRONT ENTRANCE TO THE WORK AREA.
All material is guaranteed to be as specified. Work will be performed in accordance with the drawings and specifications
submitted for above work and completed in a substantial workmanlike manner. Owner to provide electric and water at no
cost to contractor, including any special electrical set ups for swing stage "pig tails" and/or 220v service as necessary.
Contractor will remove debris and leave premises broom clean, when scrap metal need to be hauled from the premises metal
goes to the prime contractor. However, dust is an airborne nuisance and the contractor can not be held liable for dust entry
into units due to open windows by residence, broken or damaged windows, doors and other areas due to wind forces and
elements beyond the contractor's control.
Billing of actual repairs will be at the unit prices specified herein or, if not specified, equipment rental and material at cost
plus 20%, labor at $ 72.90 per man per hour, which includes: small compressors, electric hammers, grinders, drills, vacuums,
and other small tools as needed. However, despite all of our efforts in recent months to control costs, we are continuing to see
rising prices in raw material, freight, insurance, utilities and environmental expenditures. With that said unit prices might
increase due to increase in material cost.
Permit fees (if permit is required), Mobilization, Debris removal and General Conditions to be pay with first draw. Additional
mobilization and permit fees may be applied if original estimated quantities increase, some building departments requires a

total contract sum amount (AIA) at the end of the project, to charge additional fees to owners. Mobilization is a direct
reflection of the estimated quantities, in the event the contract sum increases as provided under the terms of the contract, an
additional fee of five percent (5%) of such increase shall apply for additional mobilization and/or general conditions.
Permit not usually required for maintenance type work. Any metal removed during the work and to be discarded shall be the
property of the Contractor.
Payments to be made as follows: Invoices will be submitted twice monthly based on work completed and approved by the
Engineer. Payment to be made within 7 days of engineer approval. The pay request is subject to 10 percent retention (except
on mobilization, permit fees and general conditions) on each request. The engineer shall review the application and approve it
within 7 days. If the engineer finds an objection he shall notify the contractor within 5 days. If no objections by the engineer
the approved request is sent to the owner for payment. The owner shall pay the amount approved by the engineer within 7
days. The Engineer or Management shall prepare upon completion of a "stack" or specific area a punch list of any
deficiencies in that stack or area. The contractor shall complete the punch list items before the swing stage and or scaffolding
is moved. The Engineer or Management personnel shall make a final inspection of the area and approve as completed. The
pro rata retention attributed to that completed area will be paid to the contractor. Upon substantial completion (all major
work completed and approved) all amounts invoiced under this contract including retention, shall be paid to the contractor
less an amount for "punch list" items to be done. The amount shall be determined by the engineer and/or by agreement of the
contractor and owner. Appropriate release of liens will be issued with each invoice. Payment not made to the contract will
bear 1 '/2 % interest per month. Prior to final payment the contractor shall deliver final release of liens, contractors final
affidavit, contractors warranty for the repairs will be issued upon completion of funds due to contractor.
IT IS MANAGEMENT'S AND/OR OWNER'S RESPONSIBILITY TO MOVE FURNITURE AND VEHICLES AND
OTHER PERSONAL PROPERTY OUT OF THE VICINITY OF THE WORK AREA.
THE STATE OF FLORIDA AS OF OCTOBER 1,1996 REQUIRES A NOTICE OF COMMENCEMENT BE FILED AND
RECORDED BY THE OWNER PRIOR TO ISSUING OF ANY PERMIT IN EXCESS OF $2,500.00.
ALL UTILITY LINES & PIPES IN CONCRETE AND WALL SHALL BE CLEARLY MARKED BY THE OWNER OF ITS
PRESENT. DANIELLO CONSTRUCTION CAN NOT BE RESPONSIBLE FOR CONDUITS/ OR PIPES BARED IN
CONCRETE UNLESS SPECIFIC IDENTIFIED OR MARKED. THE CONTRACTOR CANNOT BE RESPONSIBLE FOR
DAMAGE TO UNMARKED LINES & PIPES.UNLESS OTHERWISE PROVIDED FOR, Electrical hourly rate $ 115.00/11r.,
and Plumbing hourly rate $ 118.00/11r. THE CONTRACTOR WILL NOT BE HELD RESPONSIBLE FOR UNAVOIDABLE
DAMAGES, INCLUDING BUT NOT LIMITED TO EXTERIOR WALL AREAS, PAVEMENT OR LANDSCAPING.
Bushes , plants and trees must be removed from the work area , LEAD TESTING IS NOT INCLUDED IF IT'S
REQUIRED

RESPONSIBLE FOR UNAVOIDABLE DAMAGES, INCLUDING BUT NOT LIMITED TO EXTERIOR WALL AREAS,
PAVEMENT OR LANDSCAPING.
Any alteration or deviation from above specifications involving extra costs will be executed only upon written orders, and will
become an extra charge over and above the estimate. All agreements contingent upon strikes, accidents, or delays beyond our
control. Owner to carry fire, tornado, and other necessary insurance upon above work. Hurricane preparation work will be
done under time and material basis. Contractor to carry insurance as required by the State of Florida and the municipality
having jurisdiction.

Any disputes resulting from this agreement will be settled by binding arbitration under the rules of the BETTER BUSINESS
BUREAU or other arbitration remedies as the parties may decide. If legal action is required in connection with the agreement
the prevailing party shall be entitled to recover its reasonable attorney fees.
