Construction Report: Antenna Foundation Concrete Pads constructed
by United Teleports
I hereby confirm that I have reviewed the documentation contained in the attached construction
report, which includes invoices, material delivery tickets used for installation of the currently
installed concrete footings [concrete (3,000 psi), reinforcing, & anchor bolts], as well as photos
of various construction stages including construction forming, reinforcing installment, and
concrete casts.
I hereby confirm that, to the best of my knowledge and belief, and based on the attached report,
the materials and processes evidenced in said supporting documentation are in compliance with
the Engineering Drawings & Plans prepared by this firm dated 4-7-2020 (Job # 20-169) and the
current edition of the 2020 Florida Building Code.
