Report on the Construction of Antenna Foundation Concrete Pads at 10161
Range Line Road — Port Saint Lucie, 34987 — Florida
In the period from July 2016 to November 2018, our company, UNITED
TELEPORTS INC. , a lessee at the address above, has constructed 10 ( ten )
concrete foundation pads for different sizes of satellite telecommunications,
transmit and receive antennas required for our company operations in the
distribution/broadcasting of TV channels for Cable Operators and other platforms
in the USA and abroad .
All the antennas are in operations, transmitting a large number of TV channels to
Cable Operators and Direct to Home services in the US.
The list and sizes of antennas is presented in the table below.

Antenna Foundation Pads labelled as per Engineering Drawings by Jason Hanchuck PE
Besides the 10 larger transmit and receive satellite telecom antennas referred to
above, UNITED TELEPORTS also installed 10 (ten) smaller, reception only
antennas ( 3.7 m diameter ) at the back yard of the site, by means of in -ground ,
pipe type foundation.

In order to meet the wind load requirements for the region of Port Saint Lucie
West, for regulatory compliance as well as preservation of the investment in the
highly priced satellite dishes, the antenna foundation pads were implemented
with dimensions, rebars and anchor bolts exceeding the original specifications /
recommendations of the antenna manufacturers, as illustrated in the table below.

The whole process of construction of the antenna foundation pads, from concrete
forming, rebars assembly, anchor bolts assembly and concrete pouring was
directly supervised by COMLINK Inc., a company with over 30 years, worldwide
experience in the overall installation and construction of large satellite antenna:
led by Mr. Lloyd Brownhill.
Besides supervision of the construction of the concrete pads, COMLINK was it
charge of the installation of the antenna anchor bolts in accordance to the lay-(
-` required as well as of the whole build-up of the antennas.

This report was prepared to support the application 'for an "after -the -fact" permit
for the antenna foundations pads referred herein and contains the following
supporting documentation:
1) Report by COMLINK Inc. confirming that the company supervised. the
construction of the concrete pads including procedures and materials
used, as equal or exceeding the antenna manufacturers' specifications;
2) Specifications of original manufacturers of the antennas for the foundation
pads and anchors;
3) Quotes, Invoices and delivery slips for the steel rebar utilized in the
foundation pads, as evidence of types and quantities of rebar ordered for
the different pads;
4) Quotes, Invoices and delivery slips for the 3000 psi concrete utilized in the
foundation pads, as evidence of types (3000 psi) and quantities of
concrete ordered for the different pads;
5) Quotes, Invoices and delivery slips for the threaded rods, nuts and
washers utilized in the anchor bolt kits for the different antenna pads;
6) Quotes, Invoices and delivery slips for the steel tubes ordered for the in -
ground foundation of smaller, reception only antennas;
7) Pictures of the construction of the antenna pad foundations at different
stages of construction: mold framing, rebars, anchor bolt assembly and
concrete pouring, showing that materials and processes used in the
construction of the antenna foundations were consistent to the antenna
foundation plans and other documentation (invoices).
To the best of our knowledge and in accordance to the invoices and receipts
included in this report , the antenna foundation pads listed herein, were built with
dimensions and materials ( rebars ) equal or exceeding the requirements and
recommendations of the original antenna manufacturers .

1. All design and construction in accordance with the 6th Edition (2017) Florida Building Code.
2.ASCE 7-10, Risk Category III, Ultimate Design Wind Speed: 165 mph (3 second gust), Exposure C.
3. Contractor shall verify all the location of all the underground utilities prior to starting construction.
Contractor shall call 811 Sunshine for locates.
4. Contractor shall remove all vegetation, unsatisfactory soils and other deleterious material underneath
the proposed foundation.
5. Contractor shall compact the existing subgrade to 98% ASTM D 1557 Modified Proctor Maximum
Density.
6. Concrete shall have a minimum 28 day compressive strength of 3,000 psi, and water/cement ratio
shall not exceed 0.50.
7. Minimum concrete cover shall be: Slab: 3/4", Beams and columns: 1-1/2", Tie columns:'/4", Exposed
unprotected concrete: 1-1/2", Formed concrete below grade: 2", Unformed concrete below grade: 3".
8. Concrete work, including form work and testing shall be in accordance with ACI Standards, latest
edition.
9.Reinforcing bars shall be placed in accordance with CRSI Manual of Standard Practice, latest edition.
10. Reinforcing bars shall be ASTM A615 Steel and shall be free of rust and other debris.
11. Dowels shall be "L" hooked at bottom and shall be lapped 48 bar diameters with the wall or column
reinforcing above.
12. Contractor shall verify all dimensions prior to construction. Contractor shall report all irregularities
or discrepancies to the Engineer prior to proceeding with the work.
13. Foundations are designed for a minimum sail bearing value of 2,000 psf.
