This letter shall serve as certification that to the best of our knowledge,the structural repairs have been
constructed in substantial conformance with the design documents and specifications and as duly noted in
our field reports.
Threshold Inspections for the above referenced project were performed from May 5,2021 through June
15,2021. Copies of all field reports were distributed to the Client,Engineer of Record,Contractor and to
the St.Lucie County Building Department.

OBSERVATIONS:
Units 215/315,315/415&415/515
• Shoring is in place as specified on the permit drawings.
• The contractor has chipped and removed all loose concrete adjacent to the sliding glass doors.
• The deteriorated areas require a full-depth repair.
• Corrosion was found on the slab reinforcing bars as well as the top beam reinforcing under the sliding glass
door track.
• The doors were not removed,but access to the interior of the unit was necessary to place the form boards.
RECOMMENDATIONS:
Units 215/315,315/415&415/515
• Place additional reinforcing bars lapped with the existing.
• All bars shall be power tool cleaned and painted with a corrosion inhibitor.
• Form and pour repair areas with a high strength repair mortar and provide proper clearance for all
reinforcing bars.
OBSERVATIONS:
Unit 811
• There is a visible ceiling spall and cracks at the north end of the balcony edge supporting the perimeter
parapet wall.
• The contractor removed the loose stucco and exposed visible corrosion in the bottom reinforcing bars.
• We inspected the roof and parapet wall above and found(3)areas where water is ponding under the roofing
membrane,which appears to be contributing to the concrete deterioration.
RECOMMENDATIONS:
Unit 811
• Shore the balcony edge at 4-feet maximum.
• Have a licensed roofing contractor repair the roof membrane along the parapet wall.
• Repair the ceiling in 2-foot sections between post shores and relocate shores where the slab is being repaired.
All work to date is in compliance with recommendations as indicated in the permitted documents.

• All repaired areas have been poured with the approved repair mortar.
• Preparation for new waterproofing at repaired areas has begun.
• Stucco at ceiling applied at all full depth repairs.
Unit 515/615
• Tile removed at the spalled area.
• Loose concrete has been chipped at the east facing sliding glass door.
• Corrosion in the reinforcing bars does not extend into the unit.
• Spalling area is partial depth and does not require full depth repair.
Unit 811
• Roof slab edge has been repaired in the 2-foot sections as recommended.
• The extent of corrosion was limited to the north facing edge only.
• Stucco has been replaced at the repaired areas.
RECOMAMNDATIONS:
Unit 5151615
• All bars shall be power tool cleaned and painted with a corrosion inhibitor.
• Form and pour repair areas with a high strength repair mortar and provide proper clearance for all
reinforcing bars.
All work to date is in compliance with recommendations as indicated in the permitted documents.

OBSERVATIONS:
• The repaired areas have new waterproofing and are ready for paint by the painting contractor.
• Stucco applied at ceiling of full depth repairs.
• (Units 515&615)- the owners are responsible for replacing the tile of the balcony deck.
• Hurricane shutters have been re-installed and are operable.
Unit 811
• Stucco has been replaced at the repaired areas and is ready for paint.
All work to date is in compliance with recommendations as indicated in the permitted documents.
A final certification letter will follow this report.

Roof above Unit 811
We met on site with Andrew Yusko Jr.MA of Campany Roof Maintenance to discuss the roofing issue at the northeast
comer of the building.
OBSERVATIONS:
• The roofing contractor is familiar with the buildings as they have serviced them for the past 3 years.
• They removed a section of the roof above Unit 811 to explore an area in question from the roof down to the
concrete slab.
• No moisture was found under the roof membrane nor in the insulation.
• We observed the transition flashing at the parapet wall and found no areas of moisture intrusion.
• The areas of the roof that appeared to have trapped moisture under the roof membrane were determined to be
air pockets.
RECOMMENDATIONS:
• Based on the above observations,our conclusion is that there are no significant roof issues along the parapet
walls,and this did not contribute to the deterioration of the concrete ceiling below at Unit 811.
• No further action is required.
