purpos e of t his report is t o provide t he part ies of the doc um ents lis ted below w it h t he proof of int ent and v erific ation inform ation
c ollec ted at t he t im e of digital signature.
The f irs t t y pe of digit al signature is an in-person digit al s ignat ure w hich is adm inist ered in t he pres enc e of personnel ex perienc ed w it h
t he c ollec tion of digit al s ignat ures. F or t his s tyle of s ignat ure we record not only the required v erif ic at ion inf orm at ion consis ting of t he
s ignat ure ID , dev ic e I P addres s and t im es t am p t hat the s ignat ure w as c ollec t ed but als o t he nam e of the ex perienc ed digital s ignat ure
adm inist rator w ho pers onally observ ed and explained the digital signature proc es s to the c lient.
The s ec ond t y pe of digit al s ignat ure is a s end-aw ay digit al s ignat ure. I n t his t y pe of signature, t he digit al signature administ rat or
prepares a set of docum ent s to be sent and s igned on t he c lient's ow n c om puter or m obile dev ice. During t he proc es s of preparing t he
pac k age of doc ument s , t he adm inis trat or as s igns a role t o t he client and places/ac t iv at es t he correc t s ignat ure and init ial loc at ions on
t he doc um ent for t hat indiv idual. A link is then generat ed and em ailed t hat is unique to that individual that w ill begin t he review and
digital signature process.
Due to the nat ure of the s end-aw ay signatures being remot ely administ ered, t he s igner, upon rec eiving and opening t he link t o t he
doc um ent pac k age, m us t agree to the digit al s ignat ure terms and c ondit ions that indic at e t hat they hav e read and rev iew ed t he
doc um ents bef ore s igning and t hat t hey f urt her agree that t heir digital s ignat ure w ill be as legally binding as a phy s ic al pen and paper
s ignat ure. Onc e t hey have agreed to these term s and conditions , the s ignat ure request syst em will guide them t hrough t he proc es s of
rev iewing eac h page of t he doc ument pac k age bef ore init ialing or signing. F or t his type of digit al signature, t he v erific ation inform ation
c ons ist ing of signature I D, dev ic e IP addres s and timest am p is collec t ed as the s ignat ures and initials are c ompleted.
