St. Lucie County makes no representation that is granting a permit will authorize the permit holder to build the subject structure
which is in conflict with any applicable Home Owners Association rules, bylaws or and covenants that may restrict or prohibit such
structure. Please consult with your Home Owners Association and review your deed for any restrictions which may apply.
In consideration of the granting of this requested permit, I do hereby agree that I will, in all respects,perform the work
in accordance with the approved plans,the Florida Building Codes and St.Lucie County Amendments.
The following building permit applications are exempt from undergoing a full concurrency review:room additions,
accessory structures,swimming pools,fences,walls,signs,screen rooms and accessory uses to another non-residential use
WARNING TO OWNER:Your failure to Record a Notice of Commencement may result in paying twice for

Energized Electric is pleased to quote on„your new aenerator.ourchase & installation:
Energized Electric hereby agrees to furnish all labor and materials necessary for the completion of
services as specified below:
1) Supply and Install (1) 18,000-watt Generac generator model #7228 with Wi-Fi
enabled monitoring device.
2) Supply and install (1) 200-amp service entrance rated automatic transfer
switch with (2) load sharing modules if needed.
3) Price includes 55' extended service.
4) Supply preformed concrete slab. j X y
5) Deliver and set generator in place.
6) Includes labor, battery for generator, materials, and permitting.
7) Start up and test.
8) Price includes silver maintenance plan which is 2 visits. The first being at your
25-hour oil change. (Price per year after 1st year for silver maintenance is

If additional load sheds are required, they will be an additional $189.00 each
Proposal does NOT include disconnecting and reinstalling solar systems. Additional charges will
apply if solar systems are installed and fed from the load side of the automatic transfer switch.
Energized Electric recommends homeowner let the solar company know about the generator
s stem being installed
Energized Electric supplied labor and electric material are warrantied for (1) year from date of
final inspection or completion. If no permits were required, the equipment and parts are per
manufacturer's warranty polic
FLOOD ZONE PROPERTIES:
Customer is responsible for any additional cost for elevated concrete slabs and
the cost of engineering for the elevation certificate if required by building
department
$395.00 of the initial deposit is NON REFUNDABLE if customer cancels after signing this contract
or makes a deposit for any reason. No refunds will be given at all after permits have been
applied for or installation begins
A site survey of your property showing property lines is required to pull a permit. Please provide
along with this signed proposal
Customer will be responsible for getting approval from their H.O.A or P.O.A Energized will not be
liable for customer not obtaining final approval
Energized electric is not responsible for any delays due to F.P.L. or any electric service provider
for shutdowns. Energized Electric will not be responsible for any delays caused by product
shortage, availability of materials or weather delays
Price does not include a sure start or hard start if required for A/C. Additional charges will apply
if required.
