GENERAL: f % A. SCOPE: This document is the Special Threshold Inspection Plan, and is not intended to be an all- inclusive inspection checklist. The Special Inspector shall use his professional judgment, knowledge of the jobsite, and the Contractor's performance and capabilities to decide what items require additional attention. General Objectives : The�general objective of the inspection plan laid out herein is to observe the placement of structural components. This is to determine and report to the concerned parties that the structure is being built in conformance with the structural. documents, and if not, the locations and descriptions of variations. Variations from plans and specifications shall be reported, as well as actions taken by the Contractor with respect to these. Limitations: The following inspection plan is outlined as a minimum only, and may be supplemented by special requirements as noted. Contractor performance and capability may only increase Special Inspector involvement in any project. B. SPECIAL INSPECTOR: The "Special Inspector" shall be registered in Florida as a Professional Engineer, in addition to being authorized by the Florida Board of Building Codes and Standards to perform structural inspections on threshold buildings as required by Section 553.79, F.S. The "Special Inspector" may send an authorized representative to the site, but that person shall be experienced and knowledgeable in the structural systems being used. Both the actual visiting representative and the "Special Inspector" shall sign the report and all reports will be signed and sealed by the registered "Special Inspector". The Special Inspector shall not direct the Contractor's work, nor be responsible for construction means and methods. The Contractor remains solely responsible for any deviations from the Contract Documents. Should any questions arise as to the design intent, the Special Inspector shall refer such to the Architect and Structural Engineer of Record for clarification. The Special Inspector may be the engineer of record. The Special Inspector must be approved by the engineer of record. C. REPORTS : Format: The format of reports shall be as contained on the attached "Special Inspection Report" form with observations as made by the Inspector. Complete all required information. Distribution: A copy of all reports shall be forwarded to the following: 1. Owner 2. Architect 3. Enforcing Agency 4. Contractor 5. Engineer -of -Record (if other than Special Inspector) Final Report : In accordance with the State Laws, the "Special Inspector" will prepare and issue a final report upon completion of construction of the structural components which will, in effect, be a summary of the field review reports. The Special Inspector shall have performed his duties properly, to the extent that he shall file a true, signed and sealed, statement, in accordance with current State requirements, in substantially the following form: " To the best of my knowledge and belief, the above described construction of all structural load -bearing components complies with the permitted documents, and the shoring and reshoring complies with the shoring and re -shoring plans submitted to the Enforcing Agency." D. APPLICABLE DRAWINGS, SPECIFICATIONS AND REFERENCES: Prior to commencement of construction the Special Inspector will be provided with a complete set of design documents including : - structural drawings prepared by Stirling & Wilbur Engineering Group - architectural drawings - all related addenda - soils investigation report Following review and approval by the Engineer of record, the Special Inspector will be provided with approved copies of applicable shop drawings including - formwork, shoring and re -shoring plans provided by the Contractor - all approved structural shop drawings; the Special Inspector will be provided with approved copies of the following: - concrete -mix design(s) - reinforcing steel shop drawings - structural steel - miscellaneous metals - specialty items such as pre-engineered stairs. SUBSTITUTIONS / ALTERNATES / REVISIONS: Submissions for alternates as provided by the contractor or subcontractors shall be processed by the design Professionals of Record. Documentation regarding acceptance or denial of proposed alternatives shall be provided to the Special Inspector as they become available. The Special Inspector shall be advised of any structural design changes or revisions to the permitted document. II. MINIMUM REVIEW REQUIREMENTS: The following items, as a minimum, shall be inspected by the Special Inspector: A. SUBSURFACE: Al. Soils Investigation Report: All Threshold Buildings shall have had a proper soils investigation completed by a Florida - registered geotechnical P.E.'and reported on prior to the commencement of foundation installation. A copy of this report shall be filed with the Special Inspector. - confirm that notes regarding foundations on the design drawings, concur with this Soils Report. - review soils report, boring logs and foundation design recommendations. A2. Bearing Capacity - In the case of spread footings, confirm that all footings are inspected and tested by the geotechnical consultant. - verify that all pile installation is monitored and recorded by the geotechnical consultant. - verify submittal, by the geotechnical consultant, of a summary report, signed and sealed by a Florida P.E., stating that all foundations have been placed on soils capable of sustaining the specified bearing values. A3. Pilecaps and Footings : - verify that a pile location survey has been submitted and. has been approved by the Engineer of Record. - review, prior to and during concrete placement, shall include size, depth, reinforcing, clearances, cover, moisture protection. - provision for utilities / conduits - ie. - verify integrity of footings. - refer to reinforced concrete inspection requirements below. A4. Backfill and Compaction : - A qualified soils consultant shall be retained to monitor and test backfill compaction operations, and placement of slab -on -grade bases. Reports of the of the inspection and testing company will be provided to the Special Inspector. - verify testing, inspection and test results are in compliance with the contract documents. Reinforced Concrete: The Special Inspector shall be notified a minimum of 24 hours prior to the pouring of any structural concrete. Inspection shall include review of preparations prior to concrete placement, adequate inspection during placement of concrete, and verification of proper protection and treatment of concrete after placement. Prior to pour: Forms: - location and required dimensions - alignment - form -ties and spacers - formwork and re -shoring per approved shoring and re -shoring plans - openings: locations and added reinforcement where required - embedded items: size, location, chairing, secured; if conflicts - verify that no cutting or modification of detailed reinforcement is done without approval of the Engineer. - forms cleaned before pour Joints: - locations of construction, contraction and expansion joints - added bars where required - preparation of joint surface Reinforcement: - location, size, spacing, bends, laps and embedment lengths - clean - tied and supported securely - cover and clearances During pour: Concrete quality: . - materials testing per specifications - periodic check of mixing and delivery times - no water added after initial mixing Conveying and placing: - review method(s) of placement - to prevent segregation due to free drop of concrete; to prevent displacement of reinforcement and/or formwork during concrete placement. - rate of concrete placement to avoid cold joints - adequate vibration and consolidation After pour: Curing: - application and duration per specifications Strength: - strength testing. and results per specifications Repair of defective areas: - cutting, patching and curing per Engineer's directions Formwork removal: - adequate concrete strength prior to removal - formwork and shoring removal and re -shoring per approved plans. Blockwork: - verify conformance of units with specifications - check bond, alignment, plumbness, corner and end conditions - check tie beams and tie columns for location, size, spacing, reinforcement and anchorage/connection of supported masonry Reinforcement: - check filled cell locations, spacing and reinforcement - check reinforcement size, alignment, and embedment at beam/slab above - review inspection holes at bottom of filled cells prior to closing and pouring - check dowel size, location, alignment and lap length - filled cell grout : verify materials testing and results per specifications B3. Structural Steel: General: - member size, orientation and support conditions - material grade for rolled members, pipes, tubing and bolts, welding rods. - material quality - review for distortion, damage, rust, flaws, burn -holes. - surface finish - primer, galvanizing where required, finish coatings. - field touch-up of welds, scratches and abrasions. - base plates - size, thickness, anchorage, uniformity of bearing, and alignment - alignment and plumb of framing. Bolted Connections: - holes: - diameter, number, spacing, edge and end distances and misfit / corrections. - bolts: - size, grade, number, finish and tightening. - nuts and washers: - hardened, bevelled where required. Welding: - size, type, spacing, length and location. - certification of welders - prior to welding - review fitup and preparation of assembled materials. - confirm inspection and testing carried out as specified; review reports. - visual review of weld quality. - confirm repair of defective work per instructions. B4. Wood Framing: General: - review all wood framing prior to concealment by finishes. - size; grade, spacing and location of sawn lumber sections. - thickness, grade, orientation and fastening of wall and roof sheathing. - blocking over all bearings. - anchorage as specified at top and bottom of stud bearing walls. - continuity of hurricane anchor load path. - LVL or PSL lumber grade, sizes and Manufacturer, as specified. Pre-engineered Wood Trusses: - verify submittal of truss layout and truss design calculations, signed and sealed, by Truss Manufacturer. - truss type, orientation, spacing, layout, bracing and bridging per approved shop drawings - truss to truss connections per approved shop drawings. - truss end anchorage per approved anchor schedule /structural drawings. - review for cuts, notching or damage to any truss member - verify repairs in accordance with truss Manufacturer's written instructions. Miscellaneous / Specialty Items : Pre-engineered Stairs : - check installation of embedded anchors and attachments per approved shop drawings - check location and details of connections to building structure - verify general layout and framing of pre-engineered stairs conforms with approved shop drawings C. MATERIALS TESTING: All testing requirements, as defined in the structural documents, shall be carried out by an independent inspection and testing laboratory, in accordance with current Code requirements and standards of practice. Reports shall be prepared in accordance with current applicable industry standards. Copies of test reports will,be provided to the Special Inspector. The specified level of testing shall be a minimum, and the Special Inspector may direct additional tests as required. The intent of this is to allow the Special Inspector to state that the materials used are in conformance with the requirements of the structural documents.
