HWY A1A, FORT PIERCE, FL 34949 SLC BLDG. DEPT. PERMIT NUMBER: 2106-0415 PROJECT NAME: Bryn Mawr Condominiums Concrete Repairs 2021 – Bldg. C DATE: September 22, 2021 CLIENT: Elliott Merrill Community Management LOCATION: Fort Pierce, FL CONTRACTOR: Daniello & Associates, Inc. PROJECT MGR: Mr. Chris Arias INSPECTED BY: Todd Rhoten WEATHER: Sunny WORK INSPECTED: • Limited access balconies – Units 104, 201, 401, 801 & 901. OBSERVATIONS: • The Contractor set up a swing stage at the 01-balcony stack. • We accessed the balconies listed above from the swing stage and used spray paint to indicate the areas that require repairs. • The following units require repairs: o Unit 104  Hurricane shutter bottom track requires removal.  There is a large surface spall in the middle of the balcony and loose topping along the balcony edge. o Unit 201  The north edge has a ceiling crack, and requires (2) edge repairs. o Unit 401  Multiple fasteners from the old shutters remain in the slab, ceiling, and wall at west facing balcony. o Unit 801  Ceiling spalls found at east facing balcony which continue into the unit at the master bedroom. o Unit 901  Column at the north facing sliding glass door requires repair.  Multiple rust spots from fasteners in ceiling. RECOMMENDATIONS: • Unit 801 o The Owner is to remove the blinds and move the furniture away from the sliding glass doors at the master bedroom. o The Contractor is to place a temporary dust wall inside the unit. o The Contractor is to remove the hurricane shutter top track adjacent to spall and sliding glass door panels. The door track shall remain in place until the initial demo work is complete to assess the extent of corrosion. • All areas of repair shall be verified by the Engineer of Record prior to new concrete placement. ______________________________ _________________________________ Michael Lue, P.E.
