THE UNDERSIGNED HEREBY REQUEST RELEASE OF ELECTRICAL POWER TO THE ABOVE DESCRIBED
PROPERTY, FORA PERIOD NOT TO EXCEED THIRTY (30) DAYS, FOR THE PURPOSE OF TESTING SYSTEMS
AND EQUIPMENT IN PREPARATION FOR A RNAL INSPECTION, IN CONSIDERATION OF APPROVAL OF THE
REQUEST WE HEREBY ACKNOWLEDGE AND AGREE AS FOLLOWS:
1. Tills t mporary power release Is requested for the above stand purpose only, and there will be no
occupIncy of any type, other than that permitted by construction during tills time period,
2, As witness by our signatures, we hereby agree to ablde by all terms and conditions of tills agreement,
Includlhn Building I)Ivlslon Policy, which Is Incorporated herein by reference,
3. All conditions and requirements listed In the attar;te?d document entitled "Requirements for 30 Day
Power,for Testing" have been fulfilled and the promise Is ready for complianre Inspection.
4. All regUests for an extension beyond 30 days must be made In writing to the Building Official stating
the reason for (lie request. Power may be removed from the site and/or a Slop Work Order Issued if
the F*1 Inspoction has not been approved witfhln 30 clays. A fee of $a,00.00 will be required to lift
the St p Work Order,
WE HEREBY REL�ASE AND AGREE TO HOLD HARMLESS, ST, LUCIE COUNTY, AND THEIR EMPLOYEES FROM
ALL LIABILITIES IAND CLAIMS OF ANY TYPE OP NATURE WHICH MAY ARISE NOW OR IN THE FUTURE OUT
OF THIS TRANSIp{ ;CI'ION, INCLUDING ANY pAMAGE. WHICH MAY BE INCURRED DUE, TO THE
