Customer's Signature e C Printed Full Name Iza Today's Date C SCOPE OF WORK MAY 9, 2021 ALL CONSTRUCTION IMPROVEMENTS, INC. -ACI Your Window& Door Professionals Florida State Licensed & Insured CGC-058549 Removal: • Transport and distribute all units to required location. • Prepare protect and cover furniture, floor surface and work areas. • Gently score and remove existing interior and exterior sealant around all units. • Professionally remove and dispose existing unit frames, insuring minimal drywall and stucco damage. • No opening shall be left uncovered during installation process. Installation: • Unpack new units and discard packing supplies • Prepare units for installation. Inspect existing buck, reinforce or replace if necessary. • Attach new units to masonry using the required fasteners by Miami-Dade BCCO, the Florida Building Code and the International Building Code requirement. • Apply exterior Tremco Dymonic 100 polyurethane sealant. • Ensure that the SGD sill is installed with outward pitch to allow the proper drainage of water away from the structure. • Apply the polyurethane sealant as the caulking for all openings at the frame sill, jambs and header. • All openings will have new PT wood bucks where required by the NOA's or local code. • Apply interior acrylic caulk. • Patch drywall and stucco around new units. • All touch up painting is included both interior and exterior. Owner to provide the paint to match. • Daily cleanup of all work areas. -Following final inspection, install all screw caps/covers, stops and factory supplied door&window hardware. Permitting and Engineering: • Submit all required contractor specialty license and insurance documents to owner. • Prepare and file required permitting documentation: drawing and documents, wind load calculation, Miami Dade or State of Florida Product Approval documents. • Schedule inspection and assign personnel to attend final inspection day. Warranty: • Product Warranty -All materials provided by ACI are covered by the manufacturer's warranty, which shall be transferred to the Customer. -Installation Warranty - The installation is guaranteed to be performed in accordance with the permits approved by the municipal authority having jurisdiction over the Work for three (3) years from the date of final inspection. This renovation will create some dust throughout your home. Plan on covering your furniture and electronics. Please remove pictures and valuables from walls and shelves. 2. All window treatments must be taken down before installation unless special assistance needed. Our crew is not responsible for removal or replacement of window treatments and cannot be liable for damages. 3. After deposit is given a second measure will be done before your order is placed. Customer will sign off on shop drawings and window and door schedule to assure that all products are ordered correctly. 4. Design pressure labels must stay on product until final inspection is passed. 5. Product is covered by manufacturer's warranty. Installation is warranted by ACI for 36 months. 6. Product defects are warranted by manufacturer and are not subject for "non-payment" upon final inspection. 7. The following are not included in your contract and will be added to contract if needed: Rotten or damaged wood framing repair or replacement, painting, staining, window cleaning or any other unforeseen damages or circumstances. 8. If sill replacement or tile repair is necessary,ACI will provide the labor and the home owner will be responsible to provide the replacement materials. 9. Any change order or request has to be in writing signed by both parties. 10. When we deliver materials to your home, it is already paid in full by ACI to the supplier. Second payment due promptly upon delivery. The installer must collect the payment before unloading the product. We will add to the second payment any change orders (if applicable) and the reimbursement for all fees incurred for permitting. The third payment is due upon final inspection. 11. Regarding SGD's, please be aware that with all doors there is a difference in the height of the thresholds/sill riser height and this will vary based on the specific manufacturer's specifications. This is part of the door system design and its product approval as it was tested. We'll make you aware of your options in cases where it is possible to recess the doors.
