ARCHITECT'S OR ENGINEER'S AFFIDAVIT STATEMENT
THE PRODUCTS REQUIRING (FLORIDA PRODUCT APPROVAL) SUBMITTAL TO
BUILDING DEPARTMENT SHALL BE A DEFERRED SUBMITTAL, WHICH SHALL
BE SUBMITTED BY THE GENERAL CONTRACTOR. CONTRACTOR SHALL

1. ALL CONSTRUCTION SHALL BE IN COMPLIANCE WITH THE 2017, 6TH EDITION, FLORIDA BUILDING CODE WITH LOCAL
AMENDMENTS AND WITH ALL OTHER CODES, AND ORDINANCES.
2. ALL WORK RELATING TO THIS CONSTRUCTION SHALL COMPLY WITH U.S. DEPARTMENT OF LABOR, THE
OCCUPATIONAL SAFETY AND HEALTH STANDARDS AND ALL RELATED LOCAL BUILDING CODES AND ORDINANCES.
3. DO NOT SCALE THE DOCUMENTS.
4. ALL DIMENSIONS ARE TO BE FIELD VERIFIED AND BACK CHECKED FOR CORRECTNESS. IF ANY DEVIATIONS OR
DISCREPANCIES OCCUR, CONTACT THE ARCHITECT FOR VERIFICATION PRIOR TO PROCEEDING WITH THE WORK.
5. THE GENERAL CONTRACTOR AND ALL SUBCONTRACTORS SHALL CAREFULLY REVIEW THE DRAWINGS, DETAILS AND
NOTES FOR INFORMATION REGARDING THE SCOPE OF THE WORK INTENDED PRIOR TO PROCEEDING WITH THE WORK.
6. ALL MATERIALS SHALL BE INSTALLED ACCORDING TO INDUSTRY STANDARDS, ALL AGENCIES OR "STANDARD"
RECOMMENDATIONS REFERENCED IN THE SPECIFICATIONS, OR MANUFACTURERS RECOMMENDED INSTALLATION
PROCEDURES, WHICHEVER IS THE MOST STRINGENT, IN ORDER TO PROVIDE A COMPLETE AND HIGH QUALITY
PROJECT. MATERIALS, SYSTEMS OR INSTALLATIONS NOT INSTALLED PER INDUSTRY STANDARDS OR MANUFACTURERS
RECOMMENDATIONS SHALL BE REJECTED OR REMOVED.
7. THE CONTRACTOR REPRESENTS AND WARRANTS THAT IT HAS EXAMINED THE PLANS, DRAWINGS AND ALL
CONSTRUCTION CRITERIA OF OWNER AND HAS SATISFIED ITSELF THAT THE INFORMATION CONTAINED THEREIN IS
SUFFICIENT TO FULLY AND COMPLETELY CONSTRUCT THE PROJECT.
8. ALL EXTERIOR BUILDING COMPONENTS SHALL HAVE FLORIDA PRODUCT APPROVAL OR MIAMI/ DADE NOA OR
ENGINEERED SOLUTION.
9. GENERAL NOTES APPLY TO ALL DRAWINGS.
10. IT SHALL BE THE RESPONSIBILITY OF THE CONTRACTOR TO LOCATE ALL EXISTING ABOVE AND BELOW GROUND
UTILITIES WITHIN CONTRACT LIMITS & PROTECT THEM FROM DAMAGE DURING THE COURSE OF THIS CONSTRUCTION.
THE CONTRACTOR SHALL BEAR ALL EXPENSE OF, AND RESPONSIBILITY FOR REPAIR OR REPLACEMENT OF UTILITIES
OR OTHER PROPERTY DAMAGE CAUSED BY THEMSELVES OR THEIR SUBCONTRACTOR'S OPERATIONS. UTILITIES
DEACTIVATED OR ABANDONED IN PLACE BY THIS PROJECT SHALL BE CAPPED OR SEALED OFF IN ACCORDANCE WITH
LOCAL REGULATIONS.
11. CONTRACTOR SHALL SUBMIT ALL MATERIALS & COLOR SAMPLES TO OWNER FOR REVIEW AND APPROVAL PRIOR
TO ORDERING AND INSTALLATION.
12. ALL SHEET METAL AND FLASHING SHALL COMPLY WITH SMACNA ARCHITECTURAL SHEET METAL.
13. CONTRACTOR SHALL SUBMIT TO ARCHITECT ALL MARKED UP FIELD CHANGES FOR THE PRODUCTION OF RECORD
DRAWINGS AND SPECIFICATIONS.
14. CONTRACTOR SHALL SUBMIT A LIST OF PUNCHLIST ITEMS TO THE ARCHITECT FOR FINAL REVIEW AND BUILDING
PUNCH. CONTRACTOR SHALL COMPLETE PUNCHLIST ITEMS PREPARED BY THE OWNER AND ARCHITECT.
15. CONTRACTOR SHALL SUBMIT WITHIN 10 DAYS OF NOTICE TO PROCEED (NTP) A CONSTRUCTION SCHEDULE WITH
START DATE, FINISH DATE, WORK HOURS AND CONSTRUCTION PHASING PLAN IF REQUIRED AND CONSTRUCTION
HOURS TO THE OWNER FOR REVIEW AND APPROVAL.
16. CONTRACTOR SHALL BE RESPONSIBLE FOR OBTAINING AND PAYING FOR ALL APPLICABLE PERMITS AND
ADDITIONAL COPIES.
21. CONTRACTOR SHALL REVIEW WITH OWNER THEIR SAFETY/PROTECTION PLAN, STAGING PLAN, CLEANUP PLAN,
STAFFING, PORTABLE RESTROOM PLAN, AND SECURITY OF MATERIALS PLAN AT THE PRE -CONSTRUCTION MEETING.
CONTRACTOR SHALL BE RESPONSIBLE FOR ALL SITE RELATED SAFETY REQUIREMENTS.
22. CONTRACTOR SHALL NOTIFY ALL UTILITY COMPANIES AND REQUEST LOCATES PRIOR IF DIGGING IS REQUIRED OR
ANY UTILITY WORK IS REQUIRED.
23. THE SITE IS A SMOKE FREE AND DRUG FREE SITE. THERE SHALL BE NO SMOKING WITHIN 25 FEET OF ANY BUILDING
ENTRY/EXIT OR WINDOW.
24. CONTRACTOR SHALL COORDINATE PAYMENT, PAYMENT SCHEDULE, RETAINAGE AND CLOSEOUT WITH THE OWNER.
25. CONTRACTOR SHALL PROTECT ALL MATERIALS AND CONSTRUCTION FROM WEATHER, VANDELISM AND THEFT.
STANDARD UTILITY NOTES
1. ALL UNDERGROUND UTILITIES MUST BE IN PLACE, TESTED AND INSPECTED AS REQUIRED PRIOR TO BASE AND
SURFACE CONSTRUCTION.
2. THE CONTRACTOR SHALL EXERCISE EXTREME CAUTION IN AREAS OF BURIED UTILITIES AND SHALL PROVIDE AT
LEAST 48 HOURS NOTICE TO THE VARIOUS UTILITY COMPANIES IN ORDER TO PERMIT MARKING THE LOCATIONS OF
EXISTING UNDERGROUND UTILITIES IN ADVANCE OF CONSTRUCTION, BY CALLING FLORIDA SUNSHINE STATE ONE
CALL CENTER OF FLORIDA, INC. AT 1-800-432-4770. IT IS THE CONTRACTOR'S RESPONSIBILITY TO NOTIFY "SUNSHINE"
FORTY-EIGHT (48) HOURS PRIOR TO ANY CLEARING OF CONSTRUCTION TO IDENTIFY ALL UTILITY LOCATIONS.
3. IT IS THE CONTRACTOR'S RESPONSIBILITY TO NOTIFY ALL UTILITY COMPANIES TO DISCONNECT OR REMOVE THEIR
FACILITIES PRIOR TO REMOVING OR DEMOLISHING ANY EXISTING STRUCTURES FROM THE SITE. ALL UTILITIES
INCLUDING, BUT NOT NECESSARILY LIMITED TO, THE FOLLOWING SHOULD BE CONTACTED BY THE CONTRACTOR:
1. GAS
2. TELEPHONE
3. CABLE
4. POWER
5. CITY/COUNTY WATER AND SEWER
6. CITY/COUNTY/STATE TRAFFIC SIGNAL UTILITY (FIBER, HARDWIRE TRAFFIC SIGNAL INTERCONNECT)
4. THE LOCATION OF ALL EXISTING UTILITIES SHOWN ON THE PLANS HAVE BEEN DETERMINED FROM THE BEST
INFORMATION AVAILABLE. THE ENGINEER ASSUMES NO RESPONSIBILITY FOR ACCURACY. IT SHALL BE THE
CONTRACTOR'S RESPONSIBILITY TO NOTIFY THE VARIOUS UTILITIES AND TO MAKE THE NECESSARY ARRANGEMENTS
FOR ANY RELOCATIONS OF THESE UTILITIES WITH THE OWNER OF THE UTILITY. THE CONTRACTOR SHALL EXERCISE
CAUTION WHEN CROSSING ANY UNDERGROUND UTILITY, WHETHER SHOWN ON THE PLANS OR LOCATED BY THE
UTILITY COMPANY. ALL UTILITIES WHICH INTERFACE WITH THE PROPOSED CONSTRUCTION SHALL BE RELOCATED BY
THE RESPECTIVE UTILITY COMPANIES AND THE CONTRACTOR SHALL COOPERATE WITH THE UTILITY COMPANIES
DURING RELOCATION OPERATIONS. ANY DELAY OR INCONVENIENCE CAUSED TO THE CONTRACTOR BY THE VARIOUS
UTILITIES SHALL BE INCIDENTAL TO THE CONTRACT AND NO EXTRA COMPENSATION WILL BE ALLOWED.
SAFETY NOTES
1. DURING THE CONSTRUCTION AND/OR MAINTENANCE OF THIS PROJECT, THE CONTRACTOR SHALL
COMPLY WITH REGULATIONS. THE CONTRACTOR OR HIS REPRESENTATIVE SHALL BE RESPONSIBLE
FOR THE CONTROL AND SAFETY OF THE TRAVELING PUBLIC AND THE SAFETY OF ITS PERSONNEL.
LABOR SAFETY REGULATIONS SHALL BE AS SET FORTH BY OSHA IN THE FEDERAL REGISTER OF THE
DEPARTMENT OF LABOR.
2. THE MINIMUM STANDARDS AS SET FORTH IN THE CURRENT EDITION OF THE STATE OF FLORIDA,
MANUAL ON TRAFFIC CONTROL AND SAFE PRACTICES FOR STREET AND HIGH WAY CONSTRUCTION,
MAINTENANCE AND UTILITY OPERATIONS SHALL BE FOLLOWED IN THE DESIGN APPLICATION,
INSTALLATION, MAINTENANCE, AND REMOVAL OF ALL TRAFFIC CONTROL DEVICES, WARNING
DEVICES, AND BARRIERS NECESSARY TO PROTECT THE PUBLIC AND WORKERS FROM HAZARDS
WITHIN THE PROJECT LIMITS.
3. ALL TRAFFIC CONTROL MARKINGS, FLAGGERS, AND DEVICES SHALL CONFORM TO THE PROVISIONS
SET FORTH IN THE MANUAL ON UNIFORM TRAFFIC CONTROL DEVICES PREPARED BY THE U.S.
DEPARTMENT OF TRANSPORTATION FEDERAL HIGHWAY ADMINISTRATION.
4. IT SHALL BE THE SOLE RESPONSIBILITY OF THE CONTRACTOR TO COMPLY WITH AND ENFORCE ALL
APPLICABLE SAFETY REGULATIONS. THE ABOVE INFORMATION HAS BEEN PROVIDED FOR THE
CONTRACTOR'S INFORMATION ONLY AND DOES NOT IMPLY THAT THE OWNER OR ENGINEER WILL
INSPECT AND/OR ENFORCE SAFETY REGULATIONS.
CLEARING AND SITE PREPARATION NOTES
CONTRACTOR SHALL CLEAR AND GRUB ONLY THOSE PORTIONS OF THE SITE
NECESSARY FOR CONSTRUCTION.
CONTRACTOR SHALL REMOVE ALL TRASH AND UNSUITABLE DEBRIS WITHIN THE
CONSTRUCTION AREA AND PROPERLY DISPOSE OF OFFSITE.
ALL CONSTRUCTION DEBRIS AND OTHER WASTE MATERIAL SHALL BE DISPOSED OF IN
ACCORDANCE WITH APPLICABLE REGULATIONS.
CONTRACTOR TO OBTAIN ALL NECESSARY PERMITS AND APPROVALS PRIOR TO REMOVING
ANY EXISTING STRUCTURES.
IT IS THE CONTRACTOR'S RESPONSIBILITY TO NOTIFY ALL UTILITY COMPANIES TO DISCONNECT
OR REMOVE THEIR FACILITIES PRIOR TO REMOVING OR DEMOLISHING ANY EXISTING
STRUCTURES FROM THE SITE.
ALL TREE ROOTS EXISTING WITHIN APPROVED IMPROVEMENT AREAS AND
ORIGINATING FROM A PROTECTED TREE, SHALL BE SEVERED CLEAN AT THE
LIMITS OF THE WORK AREAS WHEREVER THEY ARE WITHIN 20' OF A
TREE TO BE PRESERVED.
LOCATE ALL EXISTING UTILITIES. ALL EXISTING UTILITIES SHALL BE CAPPED AND PRESERVED
FOR FUTURE TIE-INS. CONTRACTOR SHALL BE RESPONSIBLE TO NOTIFY ALL UTILITY AGENCIES
HAVING JURISDICTION PRIOR TO CONSTRUCTION.
CONTRACTOR SHALL COMPLY WITH LOCAL, STATE, AND FEDERAL
REGULATIONS REGARDING POLLUTION CONTROL OF STORED MATERIALS,
FUEL, EQUIPMENT, OR OTHER HAZARDOUS MATERIALS USED ON SITE. ALL
CONSTRUCTION DEBRIS SHALL BE STOCKPILED IN MANNER THAT DOES NOT
CAUSE SOIL, AIR, OR WATER POLLUTION ON SITE UNTIL DISPOSED OF IN A
PROPER MANNER.
CONTRACTOR SHALL COMPLY WITH SEDIMENT DISCHARGE CONTROL TO OFFSITE SO THAT
WATER QUALITY COMPLIES WITH LOCAL, STATE, AND FEDERAL TURBIDITY PARAMETERS.
CONTRACTOR SHALL SPRINKLER OR OTHERWISE APPLY WATER TO AFFECTED
CONSTRUCTION AREA TO CONTROL BOTH SIGNIFICANT WIND EROSION AND FUGITIVE DUST.
SITE NOTES
LOCATIONS, ELEVATIONS, AND DIMENSIONS OF EXISTING UTILITIES,
STRUCTURES, AND OTHER FEATURES ARE SHOWN ACCORDING TO THE BEST
INFORMATION AVAILABLE AT THE TIME OF PREPARATION OF THESE PLANS.
THE CONTRACTOR SHALL VERIFY THE LOCATIONS, ELEVATIONS, AND
DIMENSIONS OF ALL EXISTING UTILITIES, STRUCTURES AND OTHER FEATURES AFFECTING THIS WORK
PRIOR TO CONSTRUCTION.
THE CONTRACTOR SHALL CHECK PLANS FOR CONFLICTS AND DISCREPANCIES PRIOR TO CONSTRUCTION.
THE CONTRACTOR SHALL NOTIFY THE OWNER'S ARCHITECT OF ANY CONFLICT BEFORE PERFORMING ANY
WORK.
THE CONTRACTOR IS RESPONSIBLE FOR REPAIRING ANY DAMAGE TO EXISTING FACILITIES, ABOVE OR
BELOW GROUND, THAT MAY OCCUR AS A RESULT OF THE WORK PERFORMED BY THE CONTRACTOR.
IT IS THE CONTRACTOR'S RESPONSIBILITY TO BECOME FAMILIAR WITH THE
PERMIT AND INSPECTION REQUIREMENTS OF THE VARIOUS GOVERNMENTAL
AGENCIES. THE CONTRACTOR SHALL OBTAIN ALL NECESSARY PERMITS PRIOR TO CONSTRUCTION, AND
SCHEDULE INSPECTIONS ACCORDING TO AGENCY INSTRUCTION.
ALL WORK PERFORMED SHALL COMPLY WITH THE REGULATIONS AND
ORDINANCES OF THE VARIOUS GOVERNMENTAL AGENCIES HAVING
JURISDICTION OVER THE WORK.
