HWY A1A, FORT PIERCE, FL 34949 SLC BLDG. DEPT. PERMIT NUMBER: 2106-0405 PROJECT NAME: Bryn Mawr Condominiums Concrete Repairs 2021 – Bldg. A DATE: November 17, 2021 CLIENT: Elliott Merrill Community Management LOCATION: Fort Pierce, FL CONTRACTOR: Daniello & Associates, Inc. PROJECT MGR: Mr. Chris Arias INSPECTED BY: Todd Rhoten WEATHER: Overcast WORK INSPECTED: Limited access balconies – Units 102, 201, 301 & 401 OBSERVATIONS: Unit 102 • Corroded bars at the column have been exposed, power tool cleaned, coated with approved corrosion inhibitor, and are approved for concrete pour. Unit 201 • (2) ceiling spalls have been chipped and corroded bars have been exposed. • The hurricane shutter track adjacent to the ceiling spalls shall remain. • Corroded bars have been exposed, power tool cleaned, coated with approved corrosion inhibitor, and are approved for concrete pour. Unit 301 • The south door jamb is missing a wood buck and only shims are present. • A temporary weather wall has been placed at the master bedroom and the SGD has been removed. • The existing bottom track of the SGD was placed on a ½-in. high bed of mortar and is not bound to the concrete slab. • The ceiling spall at the east balcony edge has been cleaned and coated. • Corroded bars have been exposed, power tool cleaned, coated with approved corrosion inhibitor, and are approved for concrete pour. Unit 401 • The edge spall at the east balcony has been chipped of all loose concrete. • Corroded bars have been exposed, power tool cleaned, and coated with approved corrosion inhibitor. • (2) additional #4 horizontal bars have been placed along the full length of the edge repair and lap with existing. • Concrete placement is approved. • Interior blinds at the north facing SGD have been removed in preparation of the ceiling spall repair. Unit 301 • Form and pour concrete at the ceiling and surface spalls. • Contractor to provide a 1x4 PT buck at the south door jamb and fastened to the concrete wall with ¼-in. diameter Tapcons with 1 ¾” embed, spaced 12-in. on center and 4-in. from ends. Unit 401 • Form and pour concrete at edge repair. • Provide a temporary weather wall at the living room SGD. • Provide a protective barrier between the glass and the ceiling spall. • Chip loose concrete in the ceiling and chase the corrosion. • The doors will require removal if the corrosion extends beyond the door track. ______________________________ _________________________________ Michael Lue, P.E.
