St. Lucie County makes no representation that is granting a permit will authorize the permit holder to build the subject structure
which conflicts with any applicable Homeowners Association rules, bylaws or and covenants that may restrict or prohibit such
structure. Please consult with your Homeowners Association and review your deed for any restrictions which may apply.
In consideration of the granting of this requested permit, I do hereby agree that I will, in all respects, perform the work
in accordance with the approved plans, the Florida Building Codes and St. Lucie County Amendments.
The following building permit applications are exempt from undergoing a full concurrency review: room additions,
accessory structures, swimming pools, fences, walls, signs, screen rooms and accessory uses to another non-residential use
WARNING TO OWNER: Your failure to Record a Notice of Commencement may result in paying twice for
improvements to your property. A Notice of Commencement must be recorded in the public records of St.
Lucie County and posted on the jobsite before the first inspection. If you intend to obtain financing, consult

• Remove existing tile roof down to decking. Proposal;
• Replace up to 8 sheets of plywood. Renail to code with #8 ringshank nails. Additional sheets to be replaced at $95/per
sheet. Additional work to be billed @ $85/per man hour plus 15% on materials.
• Fill in skylite with 2x10(or similar), add plywood. No interior drywall, insulation or painting work is included.

• ACI to procure all necessary permits and remove all construction debris upon completion.
• A 5 year workmanship warranty, applicable metal warranty, and 30 year underlayment warranty will be forwarded upon
receipt of final payment.

Terms: This proposal becomes a Contract upon signing. Service will begin when the signed contract and a 40% de 30010 is due upon tear off of old roof and dry -in of roof. Upon substantial completion of protit is returned,

due (48 hrs) is subject to finance charges.
