F ®.1 •'` ��',,.�`' 66 Industrial Parkway "�5�11V,qL' Monticello, FL 32344 mlt Dear Mr. Charlton: I have reviewed the foundation issue at 915 Buckeye Drive, Fort Pierce, FL 34982 and have the following comments: 1. The structure is a one-story wood frame home with vertical siding and face brick, built in 1981. 2. The floor is a concrete slab on fill. 3. The foundation is a reinforced concrete footing poured integral with the floor slab. 4. The south portion of the home, and the adjacent screen porch are showing signs of settlement. 5. There are several cracks in the concrete slab floors of the southwest corner of the home.. 6. There is a vertical crack in the concrete perimeter footing of the home, due to settlement. (see photos) 7. There is significant soil erosion under the concrete slab floor of the screen porch which has created voids under the slab and is causing settlement. (see photos) 8. Measurements taken at the southeast corner of the home indicate settlement as much as 4.2 inches below the original floor. 9. The cause of the settlement is unknown, but most likely due to a combination of soil erosion and decomposing organics in the supporting soil. Alpha Foundation Specialists, Inc. Foundation Stabilization 915 Buckeye Drive Page Two 10. Soil borings were not taken. 11. It is likely that the settlement will continue to occur unless the foundation is stabilized. 12. To stabilize the foundation, we recommend installing 1 three inch helical pier at the east end of the north wall of the home, 6 three inch helical piers along the east wall of the home, 5 three inch helical piers along the east end of the south wall of the home, 3 three inch helical piers along the west end of the south wall of the home, and 3 three inch helical piers along the south end of the west of the home, for a total of 18 three inch helical piers. See attached sketch. 13. Each helical pier shall be up to 14 feet long using standard brackets and spaced a maximum of 7.0 feet as shown on the attached sketch. 14. Pier spacing may exceed 7.0 feet to avoid placement directly under a window or door or to avoid interference with HVAC or pool equipment. 15. Three foot steel spreader beams may be necessary between the bracket and bottom of footing or slab if the concrete at that point is less than eight inches thick. 16. To stabilize the floor slab of the southeast corner of the home and the screen porch, and to fill any voids under the floor slab, we recommend using PolyLEVEL as shown on the attached sketch. 17.
