Dear Mr. Harvey:
Pursuant to your request, GFA International Inc. d/b/a Universal Engineering Sciences (UES) herein provides a
review of the pile cap documentation in accordance with the threshold inspection plan prepared by the Engineer
of Record for the subject project.
Based on a review of the documents referenced above, UES has verified the following:
- A pile location survey has been submitted and has been approved by the Engineer of Record.
- Based on representations and certifications by KSM, prior to and during concrete placement of the pile
foundations, reinforcing steel, including size, depth, clearances and cover was in accordance with the
approved plans.
- Based on representations and certifications by AFT, the integrity of the pile foundations was confirmed.
Please contact the undersigned if you have any questions or comments, or if Universal Engineering may further
assist you as your plans proceed.
