This agreement confirms the installation, maintenance, and monitoring of an approved registered bait system used for
subterranean termite prevention in accordance and compliance with requirements of Florida Statutes 482.
Massey Services Inc. agrees to protect the structure noted above against the infestation of all subterranean
termites (including Formosan Termites) by utilizing the Sentricon Termite Colony Elimination System, during
and after the first year. The structure is protected by an unlimited damage repair warranty per the terms of
the guarantee. The Sentricon System will be installed after the final grade and the landscape is installed and
before the certificate of occupancy. Any concrete sla bs contiguous to the structure’s foundation will not
require a preventative treatment for termites and will not affect the structure’s warranty. The warranty and
service to the Sentricon System is for a period of 5 years following the closing date, which remains in effect
each year by the homeowner’s exercising their right to renew the service agreement and warranty. This
agreement may be extended for the lifetime of the home at the discretion of the owner and is transferable
upon the sale of the property.
SERVICE SPECIFICATIONS PROVIDED
Once installed, the Sentricon System will be monitored as outlined on the product label. A monitoring report will be
issued to the customer each service providing the following information: any new or increased termite feeding in the
bait stations added as a result of termite activity, or any addition or removal of the Recruit termite baits.
Renewal charges will be billed annually prior to the anniversary of the closing date. Payments are due net 30 days from
the date of invoice and may be provided by check or approved credit card. The annual renewal amount may be
increased after 5 years following the closing date.
